研究目的：

本研究拟通过调查问卷调查的方法，了解中国居民的心理及健康相关行为的概况，为“健康中国"策略的推进提供依据。  

Objectives of Study：

This study aims to understand the psychological and health-related behaviours of Chinese residents by means of a questionnaire to provide a basis for the promotion of the "Healthy China" strategy.

纳入标准：

①年龄≥18岁； ②具有中华人民共和国国籍； ③中国常住人口（年累计出境时间≤1个月）； ④自愿参加研究，填写知情同意书； ⑤可自行完成网络问卷调查或在调查员帮助下完成问卷调查； ⑥了解问卷每个条目所表达的含义。

Inclusion criteria

① Aged >= 18 years; ② Nationality of the People's Republic of China; ③ Permanent residence in China (cumulative annual time spent outside the country <= 1 month); ④ Voluntary participation in the study and completion of the informed consent form; ⑤ May complete the online questionnaire on their own or with the help of the investigator; ⑥ Understand the meaning expressed in each entry of the questionnaire.

排除标准：

①认知能力下降、精神异常等无法配合者； ②正在参加其他类似研究课题者； ③不愿合作者。

Exclusion criteria：

① Those who are unable to cooperate due to cognitive decline, mental abnormality, etc; ② Those who are participating in other similar research projects; ③ Those who are unwilling to cooperate.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.调查员面对面获取被调查者知情同意书后输入问卷编号，通过问卷星平台（https://www.wjx.cn/）向公众发放问 卷，调查对象通过点击链接作答。若被调查者有言语表达能力但没有足够行动能力操作答卷，则由调查员进行一 对一问询并代替其作答。 2.调查员省级负责人对调查团队所收集的问卷情况进行汇总与评价。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. The questionnaire was distributed to the public via the Questionnaire Star platform (https://www.wjx.cn/) after the investigator had obtained informed consent from the respondent face-to-face and entered the questionnaire number, and the respondent answered by clicking on the link. If the respondent was verbal but did not have sufficient mobility to operate the questionnaire, the enumerator conducted one-to-one questioning and answered on their behalf. 2. The provincial head of the investigator summarizes and evaluates the questionnaires collected by the investigation team.
