研究目的:

本项目基于前期在北京协和医院建立的GDM妊娠期动态随访队列,以及GDM母婴队列,拟联合内分泌科、儿科、妇产科、营养科协力扩大并完善协和队列,并在六家分中心(福建省立医院、南通医科大学附属医院、内蒙古医科大学附属医院、鄂尔多斯中心医院、青海省第五人民医院、宁夏医科大学附属总医院)建立多中心、跨民族妊娠期糖尿病队列。采集包括血清、血浆、脐血、胎盘、脐带、尿液、粪便、唾液、乳汁等多种类生物样本,建立生物样本库。基于上海交通大学已构建的痕量、高精度、高速度、高通量固相质谱代谢分析平台,结合液相质谱代谢分析平台及测序平台,研发多种类型生物样本组学数据采集的新技术,开发多组学时序分析方法,构建母婴代谢异常预警模型。  

Objectives of Study:

This project is based on the GDM pregnancy dynamic follow-up queue established in Peking Union Medical College Hospital in the early stage, and the GDM mother infant queue. It is planned to expand and improve the Union Medical College queue jointly with the Department of Endocrinology, Pediatrics, Obstetrics and Gynecology, and Nutrition, A multi center, cross ethnic diabetes team in pregnancy has been established in six sub centers (Fujian Provincial Hospital, the Affiliated Hospital of Nantong Medical University, the Affiliated Hospital of Inner Mongolia Medical University, Ordos Central Hospital, the Fifth People's Hospital of Qinghai Province, and the Affiliated General Hospital of Ningxia Medical University). Collect multiple types of biological samples including serum, plasma, umbilical cord blood, placenta, umbilical cord, urine, feces, saliva, milk, etc., and establish a biological sample library. Based on the established trace, high-precision, high-speed, and high-throughput solid-phase mass spectrometry metabolic analysis platform at Shanghai Jiao Tong University, combined with the liquid phase mass spectrometry metabolic analysis platform and sequencing platform, we have developed new technologies for collecting omics data from various types of biological samples, developed multiple omics time series analysis methods, and constructed a maternal infant metabolic abnormality warning model.

纳入标准:

拟纳入1000名GDM孕妇和1000名健康孕妇进行随访。 GDM孕妇纳入标准:1. 24-28周诊断GDM孕妇(75g OGTT IADPSG诊断GDM (1)FPG≥5.1 mmol/l or (2)1h≥10.0 mmol/l or (3)2h ≥8.5 mmol/l);2.包括剖腹和顺产;3.18.5

Inclusion criteria

1000 pregnant women with GDM and 1000 healthy pregnant women were included . Inclusion criteria for GDM pregnant women: 1. 24-28 weeks GDM pregnant women (75g OGTT iadpsg diagnosis GDM (1) FPG >= 5.1 mmol / L or (2) 1H >= 10.0 mmol / L or (3) 2H >= 8.5 mmol / L); 2. Including caesarean section and natural birth; 3. 18.5 < BMI < 24.0kg/m2; 4. Gdma1 & gdma2; Inclusion criteria for healthy pregnant women: 1.75g OGTT iadpsg diagnosis non GDM; 2. Including caesarean section and natural birth; 3.18.5 < BMI < 24.0kg/m2.

排除标准:

1. 双胎或多胎; 2. 合并其他妊娠期疾病; 3. 合并其他可能对组学有影响疾病,包括甲状腺疾病、炎症性肠病、肠易激综合征、乳糜泻、孕前糖尿病等; 4. 进行过胃肠道、胆系手术,包括减肥手术、胆囊切除术等; 5. 有吸烟、酗酒、麻毒药品使用史; 6. 长期用药史,服用可能影响代谢的口服药物,包括二甲双胍、阿卡波糖、倍欣、降压药、阿司匹林等; 7. 近一个月内服用影响肠道菌群的药物如抗生素等,一周内食用益生元、益生菌等食品。 健康孕妇排除标准 排除GDM高危因素包括:孕前肥胖、一级亲属患2型糖尿病、GDM史、巨大儿分娩史、合并多囊卵巢综合征、妊娠早期空腹尿糖反复阳性。

Exclusion criteria:

1. Twin or multiple; 2. Combined with other pregnancy diseases; 3. Combined with other diseases that may affect omics, including thyroid disease, inflammatory bowel disease, irritable bowel syndrome, celiac disease, pre pregnancy diabetes, etc.; 4. Had gastrointestinal and biliary surgery, including weight loss surgery, cholecystectomy, etc.; 5. Have a history of smoking, drinking and using narcotic drugs; 6. Long term medication history, taking oral drugs that may affect metabolism, including metformin, acarbose, betaine, antihypertensive drugs, aspirin, etc.; 7. Take drugs that affect intestinal flora, such as antibiotics, etc. in the past month, and eat probiotics, probiotics and other foods within one week; Exclusion criteria for healthy pregnant women High risk factors of GDM were excluded, including pre pregnancy obesity, type 2 diabetes mellitus in first degree relatives, GDM history, macrosomia delivery history, polycystic ovary syndrome, repeated positive fasting urine glucose in early pregnancy.
