主要目的：评估“H3H”营养管理模式对结直肠癌受试者营养状况的影响。 次要目的：评估“H3H”营养管理模式对结直肠癌受试者预后结局的影响。  

Objectives of Study：

Objective: To evaluate the effect of "H3H" nutrition management model on the nutritional status of subjects with colorectal cancer. Secondary objective: To evaluate the effect of "H3H" nutrition management model on the prognosis of subjects with colorectal cancer.

药物成份或治疗方案详述：

纳入标准：

1: 经过病理组织学和影像学等确诊为结直肠癌 2: 年龄>18岁 3: 预计生存时间在6个月以上 4: 需行结直肠癌局部切除或根治术 5: 接受过文化教育

Inclusion criteria

1: The diagnosis of colorectal cancer was confirmed by histopathology and imaging 2: Age > 18 years old 3: The expected survival time is more than 6 months 4: Local resection or radical resection of colorectal cancer is required 5: Have received cultural education

排除标准：

1: 依从性较差，不愿意配合本次研究 2: 正在参与其他营养干预研究 3: 自身免疫性疾病 4: 合并其他脏器的恶性肿瘤 5: 不适合人体成分测量者：安装心脏起搏器者，足踝角质层过厚者，体内有金属物质

Exclusion criteria：

1: Poor compliance and were unwilling to cooperate with this study 2: Is participating in other nutritional intervention studies 3: Autoimmune diseases 4: malignant tumors in other organs 5: Not suitable for body composition measurement: pacemaker, ankle cuticle is too thick, the body has metal substances

在常规护理的基础上加用“H3H”营养管理模式 ，主要由入院前门诊评估居家营养管理（home）+术后院内（hospital）营养管理+术后院外（home）营养管理 3个部分构成。包括建立多学科NST营养管理团队、入院后建立受试者档案、院内饮食指导和宣教、院外居家饮食和居家锻炼指导、院外营养随访。营养评估贯穿营养管理全程。

On the basis of routine nursing, the "H3H" nutrition management mode was added, which mainly consisted of three parts: pre-admission outpatient evaluation home nutrition management (home) + postoperative hospital nutrition management (hospital) + postoperative out-of-hospital nutrition management (home). It included the establishment of a multidisciplinary NST nutrition management team, the establishment of subjects' files after admission, diet guidance and education in the hospital, home diet and exercise guidance outside the hospital, and nutrition follow-up outside the hospital. Nutrition assessment runs through the whole process of nutrition management.
