研究目的：

通过文献回顾、半结构化访谈等明确居家失能老人实施自立行为的障碍因素，并据此构建失能老人家庭复能照护方案，分析基于BCW 理论的复能照护对居家失能老人上身穿衣能力、进食能力、失志状况、生活质量、抑郁等的影响，为居家长期照护服务内容的拓展提供实证依据。  

Objectives of Study：

Through literature review and semi-structured interviews, we clarify the obstacles to the implementation of self-supporting behaviors by the home-disabled elderly, and accordingly construct a reablement program for the disabled elderly, analyze the effects of reablement based on the BCW theory on the ability of the home-disabled elderly's ability to upper-body dressing ability, ability to eat, demoralization, quality of life, depression, and so on, in order to provide empirical evidence for expanding the content of the home-based long term care services.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥60周岁；②经长期护理保险失能等级评定，享受长期护理保险居家待遇的长期失能人员；③患侧上肢Brunnstrom分期为3~4期，手Brunnstrom分期为3~4期（优势侧）；④患侧上肢肌力3~4级（优势侧）；⑤坐位平衡2~3级；⑥吞咽、言语、视力、听力正常，能够配合调查研究；⑦患者自愿参与研究并签署知情同意；⑧生命体征平稳。

Inclusion criteria

① age ≥ 60 years old; ② long-term care insurance by long-term care insurance incapacity rating, enjoy long-term care insurance home treatment of long-term disabled persons; ③ the affected side of the upper limb Brunnstrom stage 3 ~ 4, hand Brunnstrom stage 3 ~ 4 (dominant side); ④ the affected side of the upper limb muscle strength 3 ~ 4 (dominant side); ⑤ seated balance of 2 ~ 3; ⑥ normal swallowing, speech, vision, hearing, able to cooperate with the study; ⑦ patients voluntarily signed the informed consent for the study; ⑧ vital signs are stable.

排除标准：

①有精神疾病或严重认知障碍；②并发其他影响上肢功能的疾病或症状，如肩关节畸形、上肢疼痛等；③仍在接受物理治疗的患者；④近1个月发生过重大个人、家庭事故，如丧亲、丧友等。

Exclusion criteria：

①mental illness or severe cognitive impairment; ②other concurrent illnesses or symptoms affecting the function of the upper extremity, such as shoulder deformity, upper extremity pain, etc.; ③patients still receiving physical therapy;④major personal and family accidents in the last month, such as bereavement or loss of a friend.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collected, including eligibility, baseline and post-intervention assessments, will be entered by two trained personnel. The data will not contain any identifying information; instead, each participant will be assigned a unique identifier generated from a sequence of random numbers on a separate form to ensure a systematic process. And only the principal investigator will be authorized to access the data. This process is designed to ensure treatment fidelity and to ensure that any confidential content is managed according to the protocol. After data entry, the two aforementioned individuals will conduct a thorough assessment of data consistency and address any omissions or inconsistencies as necessary.
