将现代康复技术与传统中医相结合，从中枢到效应器，从整体出发，综合治疗，为卒中后上肢功能障碍治疗寻求新的突破，拓展镜像视觉反馈疗法联合针刺的多模态康复模式在脑卒中后上肢功能重建的运用前景，发展中国特色康复医学，并从脑区定位、功能联结角度提供MVF联合针刺治疗的佐证。  

Objectives of Study：

The combination of modern rehabilitation technology and traditional Chinese medicine, from the center to the effector, from the whole, comprehensive treatment, seek new breakthroughs for the treatment of upper limb dysfunction after stroke, expand the application prospect of mirror visual feedback therapy combined with acupuncture multi-modal rehabilitation mode in upper limb functional reconstruction after stroke, and develop rehabilitation medicine with Chinese characteristics. It also provides evidence of MVF combined acupuncture therapy from the perspective of brain region localization and functional connection.

药物成份或治疗方案详述：

常规上肢康复训练，包括良肢位的摆放（患侧、健侧及仰卧位的，每2小时调整一次）；运动疗法（松解肩关节、主被动活动肩胛骨、诱发上肢分离运动，改善上肢异常肌张力、改善上肢运动控制，每次60min，每天1次，每周5天，共8周）及作业治疗（利用磨砂板、木棍插板等训练上肢屈伸功能和手的抓握功能，在健侧手的帮助下主动完成一些日常生活能力的动作如洗脸、刷牙、进食、穿衣等动作，每次30min，每天1次，每周5天，共8周）。增加MVF训练（20min/次，1次/天，每周6天，治疗时间为8周）。 MVF训练（20min/次，1次/天，每周6天，治疗时间为8周）。 针刺疗法（选用华佗牌一次性无菌毫针，对相关穴位进行消毒，于百会穴（平刺15-20mm）、四神聪穴（平刺13-20mm）、患侧曲池穴（直刺25-40mm）、患侧手三里穴（直刺25-40mm）、患侧内关穴（直刺13-25mm）、患侧肩髃穴（向极泉方向进针，深50-75mm）、患侧臂臑穴（直/向上斜刺20-40mm）、患侧合谷穴（直刺13-20mm）穴位针刺，平补平泻（20min/次，1次/天，每周5次，治疗时间为8周）。 

Description for medicine or protocol of treatment in detail：

Routine upper limb rehabilitation training, including good limb placement (affected side, healthy side and supine position, adjusted every 2 hours); Exercise therapy (release shoulder joint, active and passive activity of scapula, induce upper limb separation movement, improve abnormal muscle tone of upper limb, improve upper limb movement control, 60min each time, once a day, 5 days a week, a total of 8 weeks) and operational therapy (use frost-board, stick insert board, etc., to train upper limb flexion and extension function and hand grip function. With the help of the healthy side hand, I took the initiative to complete some activities of daily living ability, such as washing face, brushing teeth, eating, dressing, etc., 30min each time, once a day, 5 days a week, for 8 weeks). Increase MVF training(20min/ time, once/day, 6 days a week, treatment time for 8 weeks). MVF training (20min/ time, once/day, 6 days a week, treatment time for 8 weeks). Acupuncture therapy (use disposable sterile needle of Huatuo brand to disinfect relevant acupoints; Yu Baihui Point (Ping Acupuncture 15-20mm), Si Shencong Point (Ping acupuncture 13-20mm), Quchi Point (direct acupuncture 25-40mm), Hand Sanli Point (direct acupuncture 25-40mm), Neiguan point (direct acupuncture 13-25mm), Shoulder Condyle Point (Jiquan direction into the needle, 50-75mm deep), the affected side of the arm of the point (straight/upward oblique acupuncture 20-40mm), the affected side of the Hegu point (straight acupuncture 13-20mm) acupoint acupuncture, flat tonic and flat diarrhea(20min/ time, once a day, 5 times a week, the treatment time was 8 weeks). 

纳入标准：

①明确诊断为首次发病，发病在6月以内，病情稳定，意识清醒，伴有上肢功能障碍；②年龄 30-80 岁；③签署知情同意书；能配合完成训练及评估，依从性好；④右利手；⑤小学以上文化程度；⑥排除骨骼肌肉病变导致的上肢功能障碍；⑦患侧上肢远端关节 Brunnstrom 评定 II-IV级；⑧无认知功能障碍，简易智力状态量表(mini-mental state examination，MMSE)＞17 分。

Inclusion criteria

① The patient was diagnosed as the first onset within 6 months, stable and conscious, accompanied by upper limb dysfunction; ② Age 30-80 years old; (3) Sign informed consent; Can cooperate to complete training and evaluation, compliance is good; ④ right-handed; (5) Education level above primary school; ⑥ eliminate the upper limb dysfunction caused by skeletal muscle disease; ⑦ Brunnstrom Grade II-IV for distal upper limb joints; ⑧ No cognitive dysfunction, the score of mini-mental state examination (MMSE) > 17.

排除标准：

①患有严重高血压或心肺疾病；②严重的关节疼痛；③视力、听力及理解有严重障碍的患者；④上肢及手有畸形者。⑤双侧大脑发生病变，双侧肢体功能均有障碍，病灶位于小脑或脑干部位；⑥进展型卒中或蛛网膜下腔出血；⑦卒中后严重认知障碍、抑郁或严重失语；⑧颅脑外伤或肿瘤；⑨患者或其家属不合作，未签署知情同意书。

Exclusion criteria：

① Suffering from severe hypertension or cardiopulmonary disease; ② Severe joint pain; ③ Patients with severe impairment of vision, hearing and comprehension; ④ deformity of upper limbs and hands. (5) Bilateral brain lesions, bilateral limb function disorders, lesions located in the cerebellum or brain stem; ⑥ Progressive stroke or subarachnoid hemorrhage; ⑦ Severe cognitive impairment, depression or severe aphasia after stroke; ⑧ craniocerebral trauma or tumor; ⑨ Patients or their families do not cooperate and do not sign informed consent.
