研究目的：

1.通过随机对照试验系统性研究颞下颌关节盘不可复性前移位患者经复位钉和锚固钉复位术后颞下颌关节盘形态、位置与功能的变化； 2.探究2种开放性关节盘复位手术方式对不同患者颞下颌关节盘髁改建的差异性与特点，客观准确地评价复位钉关节盘复位术的手术效果。  

Objectives of Study：

1. A randomized controlled trial was conducted to systematically study the changes in morphology, position and function of disc reduction with reductional nails versus anchoring nails for the treatment of ADDWoR. 2. To explore the differences and characteristics of the reconstruction of the condyle of the TMJ disc in different patients with two surgery of articular disk reduction , and evaluate the effect of the reduction of the articular disk with reductional nails. .

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.18-45周岁患者； 2.MRI确诊为颞下颌关节盘不可复性前移位，关节盘长度大于等于髁突前后径； 3.病史超过3月，关节疼痛、杂音或下颌运动异常其中一项； 4.告知患者，愿意参加临床研究并签署知情同意书。

Inclusion criteria

1.Patients aged 18-45 years; 2.MRI confirmed that the temporomandibular joint disc was ADDWoR, and the length of the disc was greater than or equal to the anteroposterior diameter of the condyle; 3.A medical history of more than 3 months, one of the joint pain, murmur or abnormal jaw movement; 4.Inform patients, and they are willing to participate in clinical studies and sign informed consent.

排除标准：

1.已经行颞下颌关节紊乱病治疗患者； 2.伴有感染、肿瘤、外伤等疾病患者； 3.颌骨畸形、牙列缺损，不能形成稳定咬合，严重咬合障碍患者； 4.关节盘长度小于髁突前后径、盘严重变形、穿孔无法行关节盘手术者； 5.免疫系统疾病，患有风湿类风湿关节炎。

Exclusion criteria：

1. Patients who have been treated for temporomandibular joint disorders; 2. Patients with diseases such as infection, tumor or trauma; 3. Patients with jaw deformity, dentition defect, unable to form stable occlusion, and serious occlusion disorder; 4. The length of the joint disc is less than the anteroposterior diameter of the condyle, the disc is seriously deformed and perforated, and the operation of the joint disc cannot be performed. 5. Immune system disease, suffering from rheumatoid arthritis.

植入 1 颗复位钉，复位钉植入髁突的位置位于髁突外后方距髁突顶点下8mm处。与第一颗复位钉植入处距离3mm处再完成一次复位钉的植入。
