研究目的：

急性肾损伤（acute kidney injury, AKI）是腹部手术后常见的并发症，是指术后短时间内发生的一种肾损伤综合征，与高致残率和死亡率相关，严重影响患者的预后和生存质量。随着我国老龄人口的不断增加，接受腹部大手术的老年人数也在不断增加，术后发生AKI的比例越来越高。因此，本项目拟构建合理的 Nomogram 预测模型作为评估老年腹部大手术患者术后AKI的评估量表。利用收集的患者相关诊疗信息，通过回归模型和多因素Logistic回归模型分析，筛选和确定独立风险因素，并以此构建列线图模型。利用一致性指数评估模型预测价值并用校正曲线对模型进行验证，并用本院接受腹部手术的老年患者对模型进行外部验证，建立符合老年腹部大手术患者术后AKI的预测模型，为术后AKI高危患者提供简便、快捷、准确的评估工具，为个体化治疗提供依据。  

Objectives of Study：

Acute kidney injury (AKI) is a common complication after abdominal surgery, which refers to a renal injury syndrome that occurs shortly after surgery. It is associated with high disability and mortality rates and seriously affects the prognosis and quality of life of patients. With the continuous increase of the elderly population in our country, the number of elderly people undergoing major abdominal surgeries is also increasing, and the proportion of postoperative AKI is getting higher and higher. Therefore, this project intends to construct a reasonable Nomogram prediction model as an evaluation scale for postoperative AKI in elderly patients undergoing major abdominal surgery. Using the collected patient related diagnosis and treatment information, independent risk factors are screened and determined through regression models and multivariate logistic regression models, and a column chart model is constructed based on this. Evaluate the predictive value of the model using the consistency index and validate the model with a calibration curve. Externally validate the model with elderly patients undergoing abdominal surgery in our hospital, and establish a predictive model that is suitable for postoperative AKI in elderly patients undergoing major abdominal surgery. This provides a simple, fast, and accurate evaluation tool for high-risk postoperative AKI patients, and provides a basis for personalized treatment.

纳入标准：

(1)年龄≥65岁；(2)全麻下行Ⅰ-Ⅲ级手术。

Inclusion criteria

(1) Age ≥ 65 years old; (2) Grade Ⅰ-Ⅲ surgery under general anesthesia.

排除标准：

(1)接受心脏手术、肝移植、肺移植及部分直接影响肾脏功能的泌尿系统手术；(2)手术时间<120 min；(3)术前CKD 5期[肾小球滤过率<15 ml/(min·1.73 m2)]；(4)术前2周存在 AKI 病史；(5)术前接受过肾脏替代治疗或体外肺膜氧合治疗；(6)急诊或日间手术；(7)围产期女性；(8)术前和术后相关资料缺失。

Exclusion criteria：

(1) Accepting heart surgery, liver transplantation, lung transplantation, and some urinary system surgeries that directly affect kidney function; (2) Surgical time<120 minutes; (3) Preoperative CKD stage 5 [glomerular filtration rate<15 ml/(min · 1.73 m)] 2)]; (4) There is a history of AKI 2 weeks before surgery; (5) Received renal replacement therapy or extracorporeal pulmonary membrane oxygenation therapy before surgery; (6) Emergency or day surgery; (7) Perinatal women; (8) Preoperative and postoperative related data are missing.
