中国成人2型糖尿病患病率持续上升,目前社区常规的糖尿病健康管理仍停留在“一刀切”模式上,与患者数量激增和个性化需求增加不匹配,导致治疗控制率一直较低。文献及本课题组前期研究提示移动健康可利用、人格特质需关注。本研究拟借鉴混合方法研究思路开展整群随机社区干预试验,从策略(基于不同人格特质)、媒介(移动健康或人际网络)、内容(基于健康行动过程取向模型)三维度设计干预方案,对“分人格移动健康组”、“不分人格移动健康组”、“分人格社区人际组”3个干预组进行内容相近、方式不同的干预,对照组常规管理。通过800名患者的人格测评和6个时点追踪数据(糖化血红蛋白、行为阶段和认知心理变化等)以及典型患者深入访谈,比较各组间差异,评价在不同人格特质人群中的效果和接受度,找到适合特定人格的移动健康或传统人际传播的最佳方式,并探究行为变化的机制,最终总结出一套基于人格特质、不同媒介方式的,有针对性、可推广、可持续的社区糖尿病管理模式。  

Objectives of Study:

The prevalence of type 2 diabetes in Chinese adults continues to rise, and the current routine diabetes health management in the community is still stuck in a "one-size-fits-all" model, which is not matched with the surge in the number of patients and the increase in personalized needs, resulting in a low treatment control rate. The literature and the previous research of our research group suggest that mobile health can be used and personality traits need attention. This study intends to use mixed methods to carry out a cluster random community intervention experiment, and design intervention schemes from three dimensions: strategy (based on different personality traits), media (mobile health or interpersonal network), and content (based on HAPA model). Three intervention groups, namely "mobile health group divided with different personalities", "mobile health group with all personalities " and "community interpersonal group divided with different personalities ", were subjected to similar interventions in different ways, and the control group was routinely managed. Through the personality assessment of 800 patients and 6 time-point tracking data (HBA1c, behavioral stage and cognitive psychological changes, etc.) as well as in-depth interviews with typical patients, the differences among the groups were compared, the effects and acceptance among people with different personality traits were evaluated, and the best way of mobile health or traditional interpersonal communication suitable for specific personalities was found, and the mechanism of behavioral change was explored. Finally, a set of targeted, extendable and sustainable community diabetes management model based on personality traits and different media methods was summarized.

纳入标准:

1. 2型糖尿病诊断标准:空腹血糖≥7.0mmol/L或OGTT2小时血糖≥11.1mmol/L或HbA1C≥6.5%; 2. 年龄在18-75岁; 3. 社区常住人口(年外出时间小于1个月); 4. 入组前未服用任何精神类药物; 5. 会使用智能手机及熟悉一般微信功能; 6. 自愿参加研究,填写知情同意书。

Inclusion criteria

1. Diagnostic criteria for type 2 diabetes mellitus: fasting blood glucose >=7.0mmol/L or OGTT2 hour blood glucose >=11.1mmol/L or HbA1C>=6.5%; 2. Aged 18-75 years (note: diabetes risk group >=40 years old); 3. Permanent residents of the community (annual travel time less than 1 month); 4. Did not take any psychotropic drugs before enrollment; 5. Able to use smart phones and familiar with Wechat functions; 6. Volunteer to participate in the study and fill in the informed consent form.

排除标准:

1. 合并严重心、脑、肾、眼、足和神经系统并发症(增殖性视网膜病,肾病IV期以上或者肌酐>2mg/dl,心功能III 级以下,有脑血管意外后遗症,糖尿病足I 级以上); 2. 行动不便、神志不清、精神异常者; 3. 同时患有肿瘤,近半年内接受放疗、化疗者; 4. 正在参加其他类似研究课题者; 5. 不愿合作者。

Exclusion criteria:

1. Complicated with severe heart, brain, kidney, eye, foot and nervous system complications (proliferative retinopathy, nephropathy stage IV or above or creatinine >2mg/dl, cardiac function grade III or below, cerebrovascular accident sequelae, diabetic foot grade I or above); 2. Patients with mobility difficulties, confusion and abnormal spirit; 3. Patients suffering from tumor and receiving radiotherapy or chemotherapy within half a year; 4. Participating in other similar research projects; 5. Uncooperative.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

本研究存档数据由人工搜集的纸质数据和人工智能程序存储的电子数据文件构成,纸质数据包括研究者要求受访者填写的问卷内容、研究者。其中,纸质数据在按要求完成数据录入和核查后,按编号的顺序归档保存,并填有检索目录等,以备查考;电子数据文件包括数据库、检查程序、分析程序、分析结果、编码本和说明文件等,由微信服务平台后台生成,分类保存,并有多个备份保存于不同磁盘或记录介质上,妥善保存,防止损坏。所有原始档案按相应规定内的期限保存。

The data of this study consists of manually collected paper data and electronic data files stored by artificial intelligence programs. The paper data includes the content of the questionnaires that the researcher asked the respondents to fill out. After completing the data entry and verification, the paper data will be archived and stored in the order of numbers, and filled with retrieval catalogues for future reference. The electronic data includes databases, inspection procedures, analysis procedures, analysis results, and codebooks and description files, which will be generated by the WeChat service platform. the data will be stored in categories, and multiple backups will be stored on different disks or recording media to be properly stored to prevent damage. All original files are kept for the time limit within the corresponding regulations.
