研究目的：

旨在评估运动想象训练在提高亚急性期脑卒中患者平衡能力、降低跌倒恐惧、增强移动能力和提升功能独立性方面的有效性。  

Objectives of Study：

To evaluate the effects of motor imagery training on improving balance, reducing fear of falling, enhancing mobility and improving functional independence among subacute stroke patients.

药物成份或治疗方案详述：

所有参与者将接受标准的康复程序，包括每天30分钟的物理治疗和30分钟的职业治疗，每周五天，持续6周。物理治疗和职业治疗包括转移、平衡训练和行走等功能性训练，这些训练被融入到日常活动中。除了标准的康复治疗外，实验组还会接受每天20分钟的运动想象训练，每周五天，持续6周。每个运动想象训练环节都由研究人员指导在医院的一个安静的房间中进行。参与者将听取预先录制的音频带，音频带内容包括：1）开场放松（2分钟）；2）详细想象某些运动任务，每个任务重复两次（16分钟）。参与者将被引导视觉化完成所需任务并体验运动感知；3）注意力重新聚焦到当前环境中（2分钟）。在运动想象期间，参与者被安排躺在床上，并被要求闭上眼睛。该训练从内部视角进行，包括视觉模式（“看到”自己完成任务）和运动感知模式（“感觉”自己完成任务的体验），后者更为重要。运动想象训练结束后，研究人员会通过开放式问题询问运动想象情境的内容，以验证病人是否真正参与到运动想象中。 

Description for medicine or protocol of treatment in detail：

In this study, all participants will undergo a standard rehabilitation program, consisting of 30min of physical therapy and 30min of occupational therapy per day, five days a week for 6 weeks. Physical and occupational therapy included functional training such as transfer, balance practice, and walking, which were incorporated into daily activities. In addition to standard rehabilitation, the experimental group will receive 20-minute motor imagery training per day, five days a week for 6 weeks. Each session was individually conducted by researchers in a quiet room at the hospital. participants will listen to an pre-recorded audio tape. The audio tape will be composed of : 1) 2-min relaxation in the opening. 2) 16-min detailed imagination of some motor tasks with each task repeated for two times. participants will be guided to visualize themselves performing the required task and experience kinesthetic sensation. 3) the final 2-min refocus of attention to the immediate surroundings. During MI, participants were arranged to lie on the bed and asked to keep their eyes closed. The practice was carried out from an internal perspective, encompassing both a visual mode ("seeing" themselves performing the task) and a kinesthetic mode ("feeling" the experience of doing the task), with emphasis on the latter. After the MI sessions, researchers asked open-ended questions about the content of motor imagery sensations to verify patients’ engagement in motor imagery. 

纳入标准：

年龄在18岁以上；处于发病后2周至3个月内的脑卒中患者；处于脑卒中康复阶段，生命体征稳定；能理解并遵循简单口头指示；无任何认知损害，中文版迷你精神状态检查量表得分大于24；具有良好的运动想象能力，中文版运动和视觉想象问卷得分不低于25；中度和重度脑卒中患者，NIHSS评分在8~15或更高。

Inclusion criteria

Age greater than18; clinically diagnosed stroke patient, between 2 weeks and 3 months after stroke onset; at stroke rehabilitation stage with stable vital signs; ability to understand and follow simple verbal instructions; absence of any cognitive impairment (mini mental state examination score > 24); a good ability for motor imagery functioning, Kinesthetic and visual imagery Questionnaire score ≥25; moderate stroke and severe stroke, NIHSS score is 8-15, more than 15.

排除标准：

存在小脑或脑干神经功能缺损；有眩晕或前庭功能不全病史；存在失语症或言语障碍；存在严重视觉或听力功能障碍；存在脑损伤、脑瘤、癫痫、帕金森病等脑部疾病；当前正在参与其他研究。

Exclusion criteria：

Neurological deficits in cerebellum or brainstem; history of disease with vertigo or vestibular dysfuction; dysphasia or aphasia; severe visual or hearing dysfunction; those with brain damage, brain tumor, seizure, Parkinson’s and other brain-related diseases; those currently enrolled in other research studies.
