研究目的：

证实秦艽地黄通痹汤治疗RA活动期的临床疗效；运用代谢组学方法筛选RA活动期特异性代谢产物、探索药物作用机制并寻找疗效预测因子  

Objectives of Study：

To confirm the clinical efficacy of Gentiana in treating RA active phase, using metabolomics method, explore drug mechanism and find the predictors of efficacy

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合西医类风湿关节炎RA诊断标准患者 2.2周内急性发病，符合RA活动期评判标准的患者 3.符合中医湿热蕴毒证的RA活动期评判标准的患者 4.年龄18-60岁，男女不限 5.入组本研究患者需为既往未明确诊断为类风湿关节炎，且在发病前3月内未行相关抗风湿药物治疗 6.所有患者及其家属知晓本研究的研究目的、研究方法及参与本研究的风险，自愿参与本研究，签署相关知情同意书，并经南京中医药大学附属医院医院伦理委员会批准

Inclusion criteria

1 Patients who met the diagnostic criteria for RA in rheumatoid arthritis 2. Patients with acute onset within 2 weeks who meet the criteria for RA activity 3. Patients who meet the criteria of RA dampness and heat toxicity syndrome 4. Age is 18-60 years old, male or female 5. Patients enrolled in this study should have previously undiagnosed rheumatoid arthritis and have not received relevant antirheumatic drugs within 3 months prior to disease onset 6. All patients and their families were aware of the study purpose, study method and the risks of participating in the study, volunteered to participate in the study,signed relevant informed consent, and were approved by the Ethics Committee of the Affiliated Hospital of Nanjing University of Traditional Chinese Medicine

排除标准：

1.合并患有血清阴性脊柱关节病、皮肌炎、系统性红斑狼疮、痛风性关节炎、干燥综合征，银屑病性关节炎和硬皮病等其他风湿免疫性疾病患者 2.发病期间合并有类风湿关节炎严重关节外表现的患者 3.患者急性或慢性感染性疾病特别是化脓性关节炎患者 4.患有活动性结核病或疑似结核病患者 5.对甲氨蝶呤药物过敏患者 6.对中药汤剂过敏或者不耐受患者 7.妊娠或哺乳期妇女，试验期间或试验结束3个月内有生育计划者 8.合并严重的高血压、冠心病、脑梗、充血性心力衰竭等心脑血管系统疾病；肝肾功能不全及胃肠道疾病特别是消化道溃疡、造血系统疾病者；罹患恶性肿瘤患者 9.患有水痘患者 10.合并精神系统疾病、认知功能障碍、意识障碍、无法行有效沟通患者 11.合并先天性关节畸形，大关节既往行骨折创伤，关节置换等手术治疗的患者

Exclusion criteria：

1. Patients with seronegative rheumatic immune diseases and other rheumatic immune diseases such as spinal arthropathy, dermatomyositis, systemic lupus erythematosus, gouty arthritis, Sjogren's syndrome, psoriatic arthritis and scleroderma 2. Patients with severe extraarticular manifestations of rheumatoid arthritis during the onset of the disease 3. Patients with acute or chronic infectious diseases, especially those with septic arthritis 4. Patients with active TB or suspected TB 5. Patients allergic to methotrexate drugs 6. Patients allergic or intolerant to Chinese medicine decoction 7. Pregnant or lactating women who have birth planning within 3 months of the trial or the end of the trial 8. Patients with severe cardiovascular and cerebrovascular diseases such as hypertension, coronary heart disease, cerebral infarction and congestive heart failure; liver and kidney insufficiency andgastrointestinal diseases, especially gastrointestinal ulcer and hematopoietic diseases; patients with malignant tumors 9. Patients with chickenpox 10. Patients with mental system diseases, cognitive dysfunction, consciousness disorders, and inability to communicate effectively 11. Patients with congenital joint deformity, previous fracture trauma of large joint, joint replacement and other surgical treatment
