研究目的：

观察超声引导臂丛神经水分离治疗对nTOS患者的疗效，及nTOS患者治疗前、后胸廓出口处神经肌肉电生理变化。  

Objectives of Study：

Observation of the therapeutic effect of ultrasound-guided brachial plexus nerve water separation therapy on nTOS patients, as well as the surface electromyographic changes at the thoracic outlet before and after treatment in nTOS patients.

纳入标准：

1）以颈、肩、背及上肢疼痛不适为主诉，病程至少3个月，症状反复，不发作时完全正常，发作时痛苦不堪，诱发因素较多，寒冷、劳累、较长时间或反复同一姿势，突发上臂过度用力，突然头位改变等均可诱发症状发生或加重； （2）可伴随一些其它顽固症状（导致误诊），如顽固呃逆、胸前区疼痛、反复耳鸣、发作性头晕、头枕部疼痛、精神症状、示中指末节感觉异常等，没有严重的心血管、呼吸系统及肝肾疾病； （3）静息痛：表现为休息或睡眠前局部酸胀、疼痛不适，各部位没有缓解体位（患肢怎么放也不舒服），影响睡眠，或夜间醒来时颈肩、肢体由于无法形容的不适而无法入睡；严重者不敢平卧位，或者平躺时疼痛加重； （4）经颈椎手法按摩后，疼痛症状加重；排除其他疾病（颈肌筋膜炎、肩周炎、神经根型颈椎病）导致的疼痛或者按颈椎病、肩周炎等反复治疗，效果不佳； （5）体征：患者胸廓出口处（包括：锁骨上斜角肌三角和/或在锁骨下胸小肌腱处）明显压痛，可伴同侧肩部、上肢的放射痛； （6）愿意配合研究者，并清楚的了解本次临床研究的目的，并愿意签署知情同意书

Inclusion criteria

1) The main complaint is neck, shoulder, back and upper limb pain and discomfort, the course of the disease is at least 3 months, the symptoms are repeated, completely normal when they do not occur, and the pain is unbearable when they occur. There are many inducing factors, such as cold, fatigue, prolonged time or repeated posture, sudden overexertion of the upper arm, sudden change of the head position, etc., can induce the occurrence or aggravation of symptoms; (2) It can be accompanied by some other stubborn symptoms (leading to misdiagnosis), such as stubborn singultus, pain in the chest area, repeated tinnitus, seizure dizziness, head and pillow pain, mental symptoms, indicating the middle finger end paresthesia, no serious cardiovascular, respiratory system and liver and kidney diseases; (3) Resting pain: manifested as local swelling, pain and discomfort before rest or sleep, no relief position in each part (how to put the affected limb is uncomfortable), affecting sleep, or wake up at night, neck, shoulder and limb can not fall asleep due to unspeakable discomfort; Severe patients do not dare to lie flat, or the pain is aggravated when lying flat; (4) After cervical manual massage, the pain symptoms worsened; Exclude the pain caused by other diseases (neck myofasciitis, shoulder periarthritis, nerve root type cervical spondylosis) or according to cervical spondylosis, shoulder periarthritis and other repeated treatment, the effect is not good; (5) Signs: The patient has significant tenderness at the thoracic outlet (including the supraclavicular scalene triangle and/or at the subclavian pectoral minor tendon), which may be accompanied by radiating pain in the ipilateral shoulder and upper limb; (6) Willing to cooperate with researchers, clearly understand the purpose of this clinical study, and willing to sign informed consent

排除标准：

（1）严重的意识、运动、认知、情感障碍，影响了本临床研究的执行或者测评，无法进行积极康复治疗者； （2）肝炎、活动性结核、局部金属异物、妊娠、青光眼、骨肿瘤，封闭部位皮肤感染或者皮肤病变； （3）既往有麻醉药物过敏、激素类药物过敏、消化道出血病史，出血倾向； （4）行走不稳，考虑脊髓型颈椎病者； （5）对局部注射治疗异常担心，不愿意签署治疗知情同意书的患者

Exclusion criteria：

(1) Serious consciousness, motor, cognitive, emotional disorders, affecting the implementation or evaluation of this clinical study, unable to carry out active rehabilitation treatment; (2) hepatitis, active tuberculosis, local metal foreign bodies, pregnancy, glaucoma, bone tumors, skin infections or skin lesions at closed sites; (3) Previous history of anaesthetic drug allergy, hormone drug allergy, gastrointestinal bleeding, bleeding tendency; (4) Walking instability, consider cervical spondylotic myelopathy; (5) Patients who are abnormally worried about local injection therapy and are unwilling to sign informed consent for treatment

①：口服肌松药（乙哌立松）每日3次，每次50mg；②口服改善微循环药（地巴唑）每日3次，每次10mg，神经营养药（甲钴胺）每日3次，每次500ug；③口服COX-2 抑制剂（塞来昔布）每日1次，每次200mg。

由深圳市前海蛇口自贸区医院病案科独立统计学专家使用采用SPSS20.0版软件产生随机数字号码及随机分组，并且填写随机分配卡，包含序号、随机号码及组别。将随机分配卡密封装入不透光的专用信封中，然后在信封外面编写序号。由专人进行随机管理和操作，随机操作人员不参与疗效评价和治疗或统计分析，在合格受试者接受针臂丛神经水分离治疗前，医师通过随机操作人员获得随机信息及分组。
