研究目的：

（1）了解出院后脑卒中患者家庭主要照顾者的自我调节疲劳水平、影响因素及相关症状体验，探索脑卒中患者家庭主要照顾者的自我调节疲劳相关症状类型，通过一对一面谈，解释性理解脑卒中患者家庭主要照顾者在长期照护中的体验； （2）分类脑卒中患者出院后的不同时期，不同家庭成员身份的照顾者、不同角色适应类型，并描绘家庭主要照顾者自我调节疲劳相关症状地图，探索家庭主要照顾者照护过程中自我调解疲劳水平的发展趋势； （3）基于脑卒中患者家庭主要照顾者自我调节疲劳相关症状规律及相关需求，构建脑卒中患者家庭主要照顾者症状精准化管理方案并进行可行性分析，提升家庭照顾者自我控制能力及脑卒中患者延续性护理质量，促进脑卒中患者家庭康复健康管理的建设。  

Objectives of Study：

(1) Understand the self-regulated fatigue levels, influencing factors, and related symptom experiences of the primary caregivers of stroke patients after discharge, explore the types of self-regulated fatigue related symptoms of the primary caregivers of stroke patients, and explain the experience of the primary caregivers of stroke patients in long-term care through one-on-one interviews; (2) Classify stroke patients at different stages after discharge, caregivers with different family member identities, and different role adaptation types, and depict a map of self-regulation fatigue related symptoms of primary caregivers in the family, exploring the development trend of self-regulation fatigue levels during the care process of primary caregivers in the family; (3) Based on the self-regulation fatigue related symptoms and related needs of the main caregivers of stroke patients, a precise symptom management plan for the main caregivers of stroke patients is constructed and feasibility analysis is conducted to improve the self-control ability of family caregivers and the quality of continuous care for stroke patients, and to promote the construction of family rehabilitation and health management for stroke patients.

纳入标准：

脑卒中患者纳入标准： （1） 符合脑卒中诊断标准，经脑CT或MRI确诊； （2） 改良Barthel指数评分<60分； （3） 居住地为衡阳市城区内的社区； （4） 无理解和阅读障碍； （5） 自愿加入本次研究者； 家庭主要照顾者纳入标准： （1） 照顾脑卒中患者时间最长、承担照顾任务最多者； （2） 年满18周岁且意识清楚； （3） 能理解问卷内容并准确表达者； （4） 知情同意，愿意配合者

Inclusion criteria

Inclusion criteria for stroke patients: (1) Meets the diagnostic criteria for stroke and is diagnosed through brain CT or MRI; (2) Improved Barthel index score<60 points; (3) Residing in a community within the urban area of Hengyang City; (4) No comprehension and reading barriers; (5) Voluntarily join the researcher for this study; Inclusion criteria for primary family caregivers: (1) Those who take care of stroke patients for the longest time and undertake the most care tasks; (2) At least 18 years old and conscious; (3) Can understand the questionnaire content and accurately express it; (4) Informed consent and willingness to cooperate

排除标准：

脑卒中患者排除标准： （1） 伴随其他严重疾病、恶性肿瘤、终末期、透析患者； （2） 间歇性精神异常者 （3） 出院时BI值>60分。 脑卒中患者主要照顾者排除标准： （1） 需支付报酬者； （2） 经过专业照护培训者； （3） 存在精神病史者； （4） 因患有严重疾病无法照顾者； （5） 家庭近期遭受其他重大应激事件者。

Exclusion criteria：

Exclusion criteria for stroke patients: (1) Accompanied by other serious diseases, malignant tumors, end-stage, and dialysis patients; (2) Intermittent mental disorders (3) At discharge, the BI value was greater than 60 points. Exclusion criteria for primary caregivers of stroke patients: (1) Those who need to pay remuneration; (2) Trainees who have undergone professional care training; (3) Individuals with a history of mental illness; (4) Those who are unable to take care of due to serious illnesses; (5) Individuals whose families have recently experienced other major stress events.
