研究目的：

1.通过超声监测评估休克病人肠系膜循环血流动力学，观察不同肠系膜血流动力学病人预后，进行流行病学调查。 2.通过胃肠超声联合肠道屏障生化指标，寻找合适的肠内营养时机,以期改善休克患者的预后。  

Objectives of Study：

1. Evaluate the mesenteric circulation hemodynamics of shock patients through ultrasound monitoring, observe the prognosis of patients with different mesenteric hemodynamics, and conduct epidemiological investigations. 2. By combining gastrointestinal ultrasound with intestinal barrier biochemical indicators, suitable timing for enteral nutrition is sought to improve the prognosis of shock patients.

药物成份或治疗方案详述：

入组病人按喂养流程喂养，初始速度0.5ml/kg/h。喂养流程：如果没有肠内营养禁忌症，则始肠内喂养。按流程图实施肠内营养，直到肠内营养输注速率达到目标（1.5ml/kg/h）。我们设定了第一周的目标热量摄入为20-30千卡/公斤/天，在喂养不耐受的情况下补充肠外营养。患者将在肠内营养前、肠内营养后4-6小时、24小时后进行三次重症超声检查（包括肠系膜上动脉、门静脉、下腔静脉、肝静脉、肾静脉超声测量数据），在肠内营养前、肠内营养后24小时后我们将使用剩余的血液标本进行两次肠道屏障生化指标检查（二胺氧化酶、D-乳酸、内毒素）。 

Description for medicine or protocol of treatment in detail：

The enrolled patients were fed according to the feeding process, with an initial speed of 0.5ml/kg/h. Feeding process: If there are no contraindications to enteral nutrition, enteral feeding should be started. Implement enteral nutrition according to the flowchart until the enteral nutrition infusion rate reaches the target (1.5ml/kg/h). We set the target calorie intake for the first week at 20-30 kcal/kg/day, with parenteral nutrition supplementation in the case of feeding intolerance. The patient will undergo three severe ultrasound examinations (including ultrasound measurements of the superior mesenteric artery, portal vein, inferior vena cava, hepatic vein, and renal vein) before enteral nutrition and 4-6 hours and 24 hours after enteral nutrition. Before enteral nutrition and 24 hours after enteral nutrition, we will use the remaining blood samples to perform two intestinal barrier biochemical index tests (diamine oxidase, D-lactate, endotoxin). 

纳入标准：

1、休克患者，血流动力学稳定（血管活性药物剂量稳定或减量时且去甲肾上腺素≦0.5ug/kg/min）拟开启肠内营养； 2、年龄大于等于18岁； 3、入住ICU 24小时以上，NRS-2002≥3； 4、已签署知情同意书。

Inclusion criteria

1. Shock patients with stable hemodynamics (when the dosage of vasoactive drugs is stable or reduced, and norepinephrine is ≤ 0.5ug/kg/min) plan to initiate enteral nutrition; 2. Age 18 years or older; 3. Stay in ICU for more than 24 hours, NRS-2002 ≥ 3; 4. Signed informed consent form.

排除标准：

1、存在肠内营养禁忌症（未控制的休克，未控制的低氧血症与酸中毒，活动性消化道出血，肠缺血与肠梗阻，腹腔间隔室综合征，无远端喂养通道的高流量肠瘘）； 2、既往有慢性胃肠道疾病（如克罗恩病、溃疡性结肠炎）或因腹部手术入住ICU； 3、慢性严重肝病、慢性肾功能衰竭，晚期肿瘤； 4、妊娠或哺乳； 5、多脏器功能衰竭，脑死亡。

Exclusion criteria：

1. There are contraindications for enteral nutrition (uncontrolled shock, uncontrolled hypoxemia and acidosis, active gastrointestinal bleeding, intestinal ischemia and obstruction, abdominal compartment syndrome, high flow intestinal fistula without distal feeding channels); 2. Previous chronic gastrointestinal diseases (such as Crohn's disease, ulcerative colitis) or admission to the ICU due to abdominal surgery; 3. Chronic severe liver disease, chronic renal failure, advanced tumors; 4. Pregnancy or lactation; 5. Multiple organ failure, brain death.
