研究目的：

（1）构建一套适用于老年肌少症住院患者的营养运动干预方案； （2）探讨干预方案的可行性和安全性，分析其对老年肌少症住院患者肌肉质量、肌肉力量、活动能力以及生活质量的影响。  

Objectives of Study：

(1) Develop a nutritional exercise intervention plan suitable for hospitalized elderly patients with sarcopenia; (2) Explore the feasibility and safety of intervention plans, and analyze their impact on muscle mass, muscle strength, activity ability, and quality of life of hospitalized elderly patients with sarcopenia.

纳入标准：

（1）纳入标准 ①符合 2019 版亚洲肌少症诊断策略： a 肌肉力量：最大握力男性＜28kg，女性＜18kg； b 躯体功能：5 次起坐试验≥12s； c 骨骼肌含量：以上下肢骨骼肌质量指数（SMI）为准，计算方法为四肢骨骼肌质量（kg）与身高的平方（m）的比值，生物电阻抗法（BIA）诊断范围为男性＜7.0kg/㎡，女性＜5.7kg/㎡。 骨骼肌含量降低+肌肉力量降低/躯体功能下降者，视为肌少症。 ②年龄 ≥60 岁； ③BI 指数 ≥40 分； ④无语言沟通障碍、可配合完成量表评估者； ⑤既往无规律运动习惯； ⑥患者及家属知情同意，自愿加入。

Inclusion criteria

(1) Inclusion criteria ① Compliant with the 2019 diagnostic strategy for Asian sarcopenia: Muscle strength: maximum grip strength for males<28kg, females<18kg; Physical function: 5 sit up tests for ≥ 12 seconds; Skeletal muscle content: The lower limb skeletal muscle mass index (SMI) is used as the standard, and the calculation method is the ratio of limb skeletal muscle mass (kg) to the square of height (m). The diagnostic range of bioelectrical impedance (BIA) is less than 7.0kg/m2 for males and less than 5.7kg/m2 for females. A decrease in skeletal muscle content, muscle strength, or physical function is considered sarcopenia. ② Age ≥ 60 years old; ③ BI index ≥ 40 points; ④ Individuals who have no language communication barriers and can cooperate in completing scale assessments; ⑤ Previous irregular exercise habits; ⑥ The patient and their family members voluntarily join with informed consent.

排除标准：

（2）排除标准 ①体内装有心脏起搏器、接受过人工关节置换或截肢手术患者； ②伴有不可逆转的严重并发症或恶性肿瘤、重要脏器功能衰竭等重症患者； ③内生肌酐清除率（Ccr）少于 30ml/min，平常对蛋白质摄入量需严格控制的患者。

Exclusion criteria：

(2) Exclusion criteria ① Patients with a pacemaker installed in their body, who have undergone artificial joint replacement or amputation surgery; ② Severe patients with irreversible complications or malignant tumors, important organ failure, etc; ③ Patients who have an endogenous creatinine clearance rate (Ccr) of less than 30ml/min and require strict control of protein intake.

干预组在接受常规干预的基础上接受GCS评估和营养-运动干预：干预内容详见研究计划书表1、表2。住院期间由护士指导进行干预，回家后由患者自行进行锻炼，护士定期随访。

干预措施：

住院期间给予患者常规运动、饮食指导，干预频率为每周一次，包括：a 饮食清淡为主，少食多餐，避免过度饥饿或过度饱腹，增加含钙高的食物及植物性蛋白质（如大豆、牛奶）的摄入，多吃新鲜蔬菜水果，减少高胆固醇食物；b 体力低下的老人选择合适的辅助工具，如进行慢走、散步等运动方式。护士一周一次向患者进行健康宣教，每月举行一次相关主题讲座。

During hospitalization, patients are given routine exercise and dietary guidance, with an intervention frequency of once a week, including: a Focus on a light diet, eat fewer and more meals, avoid excessive hunger or satiety, increase intake of high calcium foods and plant-based proteins (such as soybeans and milk), eat more fresh vegetables and fruits, and reduce high cholesterol foods; Elderly people with low physical strength should choose appropriate assistive tools, such as slow walking, walking, and other forms of exercise. Nurses provide health education to patients once a week and hold related themed lectures once a month.
