研究目的：

本研究针对GDM高危人群，在孕早期以计划行为理论（theory of planned behavior，TPB）为框架构建干预措施，实施健康教育以提高孕妇对妊娠期糖尿病的认识，全面了解其高危因素，促进主动遵循其高危因素管理；针对性讲解个体具有高危因素相关的危害、治疗和改善等知识。借鉴“五驾马车”的治疗手段，对GDM 的高危因素进行干预，从而达到控制GDM发生的目的。由此，本研究干预措施共分两大部分，预防DM、GDM采取生活方式的干预；针对GDM的高危因素，如甲状腺疾病、高脂血症、乙型肝炎、妊娠期VVC等，进行针对性干预。妊娠期糖尿病高危因素的日志式管理是将“GDM 一日门诊”等成熟的管理模式应用于GDM高危因素的孕妇，早期筛查高危因素并干预，观察孕中、晚期GDM发病率。尽可能做到早教育、早识别、早预防、早发现、早诊断、早管理、早治疗，探索教育内容、方法及效益评价，进一步改善孕产妇结局及围产儿的生存质量。  

Objectives of Study：

In this study, we aimed at GDM high-risk groups, and constructed interventions based on theoryofplannedbehavior (TPB) in early pregnancy. Health education was conducted to improve the awareness of pregnant women with gestational diabetes mellitus, to understand their risk factors comprehensively and to actively follow their high-risk factors management. Specifically explain that individuals have knowledge of hazards, treatment and improvement related to high-risk factors. Learn from the treatment means of "five carriages" to intervene the high-risk factors of GDM, so as to control the occurrence of GDM. Therefore, the intervention measures in this study are divided into two parts: preventing DM and GDM and taking lifestyle intervention; Targeted intervention should be carried out for the high-risk factors of GDM, such as thyroid disease, hyperlipidemia, hepatitis B, VVC during pregnancy, etc. The management of high-risk factors for gestational diabetes is to apply the mature management mode such as GDM day clinic to GDM high-risk women, screening high-risk factors and intervening early, and observing the incidence rate of GDM in the second and third days of pregnancy. Try to achieve early education, early identification, early prevention, early detection, early diagnosis, early management and early treatment, explore educational contents, methods and benefit evaluation, and further improve maternal outcomes and the quality of life of perinatal infants.

纳入标准：

1.孕早期于北京市大兴区人民医院建档、定期产检的孕妇；
2.单胎妊娠孕妇；
3.入组孕周＜14周；
4.具有以下一项或多项高危因素：年龄＞35岁、BMI≥24、糖尿病家族史、高血压家族史、既往分娩巨大儿史、既往妊娠期糖尿病史、既往妊娠期高血压疾病史、高血压病史、尿糖阳性、高脂血症、甲状腺功能减退、多囊卵巢综合征、孕前月经不规律、外阴瘙痒伴假丝酵母菌感染；
5.通过早期筛查排除孕前糖尿病或糖耐量异常；
6.同意签署书面知情同意书并愿意配合治疗。

Inclusion criteria

1. Pregnant women who are registered in Daxing District People's Hospital of Beijing in the early stage of pregnancy and have regular prenatal examination;
2. Pregnant women with single pregnancy;
3. The gestational weeks in the group were less than 14 weeks;
4. Have one or more of the following high-risk factors: age>35 years old, BMI ≥ 24, family history of diabetes, family history of hypertension, previous history of macrosomia, previous history of diabetes in pregnancy, previous history of hypertension in pregnancy, history of hypertension, positive urine glucose, hyperlipidemia, hypothyroidism, polycystic ovary syndrome, irregular menstruation before pregnancy, pruritus vulvae with candida infection;
5. Eliminate pre pregnancy diabetes or abnormal glucose tolerance through early screening;
6. Agree to sign a written informed consent and be willing to cooperate with the treatment.

排除标准：

1.双胎及多胎妊娠；
2.精神异常不能配合者；
3.妊娠期服用可能影响糖代谢及脂代谢药物者；
4.有活动性肝炎或乙肝病毒携带者；
5.有心脏病、肾病、呼吸系统疾病、免疫系统疾病、结缔组织病等慢性病史；
6.依从性差，不能配合干预指导，如听课次数＜1次。

Exclusion criteria：

1. Twin and multiple pregnancy;
2. Those who are mentally abnormal and unable to cooperate;
3. Those who take drugs that may affect glucose metabolism and lipid metabolism during pregnancy;
4. Active hepatitis or hepatitis B virus carrier;
5. Have a chronic history of heart disease, kidney disease, respiratory system disease, immune system disease, connective tissue disease and other diseases;
6. Poor compliance, unable to cooperate with intervention and guidance, such as the number of lectures<1.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

采用通过阅读文献后自行设计的调查表（妊娠期糖尿病相关高危因素的问卷调查），由本 人负责收集相关临床资料。调查内容主要包括人口社会学资料、生活环境、饮食情况、家族 史、既往疾病史、月经生育史、妊娠期相关检测指标和围产期结局等，收集问卷并由专人管 理。于表 1规定时间内完成听课后的知识水平问卷。产前随访于产科门诊完成，同时收集电 子病历系统及校验数据。孕中晚期收集 OGTT结果。由专人录入数据，另一人核对录入的数 据是否正确，同时收集电子病例系统的数据。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

I used the self-designed questionnaire (reading diabetes related risk factors questionnaire) after reading the literature, and I was responsible for collecting relevant clinical data. The survey mainly includes demographic and sociological data, living environment, diet, family history, past disease history, menstrual and reproductive history, pregnancy related test indicators and perinatal outcomes. Questionnaires are collected and managed by special personnel. Complete the knowledge level questionnaire after class within the time specified in Table 1. Prenatal follow-up was completed in the obstetric clinic, and the electronic medical record system and calibration data were collected at the same time. OGTT results were collected in the second and third trimester of pregnancy. A specially assigned person shall input the data, and another person shall check whether the input data is correct, and collect the data of the electronic case system at the same time.
