研究目的：

构建科学、标准、系统的糖尿病分阶段自我管理支持干预方案，并对该干预方案的可行性、可接受性以及初步效果进行评价，以期为糖尿病管理提供参考。  

Objectives of Study：

To construct a scientific, standardized and systematic phased self-management support intervention program for type 2 diabetes, and to evaluate the feasibility, acceptability and preliminary effects of the intervention program with the aim of providing a reference for diabetes management.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合中国2型糖尿病防治指南（2020年版）诊断标准的2型糖尿病患者； ②年龄为18～75岁； ③近三个月HbA1c≥7.5%； ④目前没有使用胰岛素或胰岛素类似物进行治疗； ⑤未接受过系统化糖尿病健康教育课程； ⑥同意参加本研究，并签署知情同意书。

Inclusion criteria

① Patients with type 2 diabetes who meet the diagnostic criteria of the Chinese Guidelines for the Prevention and Treatment of Type 2 Diabetes (2020 Edition); ②Age is 18~75 years old; ③ HbA1c≥7.5% in the past three months; ④no current treatment with insulin or insulin analogues; ⑤ Have not received systematic diabetes health education courses; ⑥ Agree to participate in this study and sign an informed consent form.

排除标准：

①怀孕或准备怀孕者； ②行动不便、有精神异常、认知障碍者、不能合作者； ③合并严重糖尿病并发症； ④肿瘤患者，近半年内接受放疗、化疗者； ⑤正在参加其他类似研究课题的患者。

Exclusion criteria：

①Those who are pregnant or planning to become pregnant; ② People with reduced mobility, mental abnormalities, cognitive impairments, and those who cannot cooperate; ③ combined with serious diabetic complications; ④Tumor patients, who have received radiotherapy and chemotherapy in the past six months; ⑤Patients who are participating in other similar research topics.

采用2型糖尿病患者自我管理支持干预方案进行干预。第一阶段为结构化教育课程，为期一个月（项目前期构建）；第二阶段为持续性自我管理支持干预方案（本研究第一部分构建），分别在第三个月、第六个月、第九个月进行强化支持，具体实施主要由研究者本人进行，并在基线、干预中及干预后评估方案可行性、可接受性及初步效果。

The intervention was carried out using a self-management support intervention programme for people with type 2 diabetes. The first stage is a structured education course (which has been built and verified by the project team in the early stage), which lasts for one month; The second stage is the continuous self-management support intervention program (constructed in the first part of this study), which is supported by intensive support in the third month, sixth month, and ninth month, and the specific implementation is mainly carried out by the investigators themselves, and the feasibility, acceptability and preliminary effect of the program are evaluated at baseline, during and after the intervention.
