研究目的：

基于MPS 的发生机制与前期临床预试验结果，我们拟研发一种超声引导下肌筋膜水分离新技术(Ultrasound-guided Myofascial Hydrodissection Technique, UMHT)。通过手法触诊定位受累骨骼肌，采用单纤维sEMG技术靶向识别MTrP；在超声引导下对MTrP及周围肌筋膜系统注入适量生理盐水，逐层松解MTrP周围的肌周膜、肌束膜和肌外膜；最终达到灭活MTrP、整复肌筋膜功能和缓解疼痛的作用。  

Objectives of Study：

Based on the pathogenesis of MPS and the results of preliminary clinical trials, we proposed to develop a new Ultrasound-guided Myofascial Hydrodissection Technique (UMHT). The affected skeletal muscle was located by manual palpation, and MTrP was identified by single fiber sEMG technique. Under the guidance of ultrasound, appropriate amount of normal saline was injected into MTrP and the surrounding myofascial system, and the perimuscular, fascicular and epimyosal membranes around MTrP were relieved layer by layer. Finally, it can inactivate MTrP, restore myofascial function and relieve pain.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 20~70 岁。 (2) 符合《肌筋膜疼痛综合征的诊断与治疗》中关于MPS的诊断标准：满足5个主要标准和至少1个次要标准： 主要标准： ①主诉区域性疼痛； ②主诉疼痛或触发点牵涉痛的预期分布区域的感觉异常； ③受累肌肉可触及紧张带； ④紧张带内的某一点呈剧烈点状触痛； ⑤在测量时，存在某种程度的运动受限。 次要标准： ①压痛点重复出现主诉的临床疼痛或感觉异常； ②触诊带状区诱发局部抽搐反应。 (3) 采用单纤维sEMG检查。

Inclusion criteria

(1) 20 to 70 years old. (2) Meet the diagnostic criteria for MPS in the Diagnosis and Treatment of Myofascial Pain Syndrome: meet 5 major criteria and at least 1 minor criteria: Main criteria: ① complained of regional pain; (2) paresthesia in the expected distribution area of the chief complaint of pain or the trigger point of referred pain; ③ The affected muscle can reach the tension band; ④ a point in the tension zone is intense point-like tenderness; ⑤ At the time of measurement, there is some degree of limited movement. Secondary criteria: ① Recurrent clinical pain or paresthesia at the tender point; ② Local convulsive reaction was induced by palpation of the ribbon area. (3) Single fiber sEMG test was adopted.

排除标准：

(1) 孕妇或备孕； (2) 诊断为椎间盘突出症引起上肢/下肢放射性疼痛； (3) 脊柱感染、骨折及肿瘤； (4) 类风湿性关节炎、强直性脊柱炎活动期；

Exclusion criteria：

(1) Pregnant women or pregnant women; (2) Radiation pain in the upper/lower limbs due to disc herniation; (3) spinal infection, fracture and tumor; (4) Rheumatoid arthritis, ankylosing spondylitis active period;
