研究目的：

本研究通过检测两组患者循环中内皮细胞损伤标志物和糖萼脱落成分以及血浆炎症指标浓度和血流剪切力的变化，探讨术前房颤对围术期血管内皮细胞和糖萼的影响，旨在加强房颤管理并积极改善预后。  

Objectives of Study：

This study examined the effects of preoperative atrial fibrillation on vascular endothelial cells and glycocalyx in the perioperative period by detecting the changes in endothelial cells injury markers and glycocalyx shedding components, plasma inflammation factor concentrations and blood flow shear forces in the two groups of patients , aims to strengthen the management of AF and improve the prognosis actively.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)于我院择期体外循环下行二尖瓣置换或成形的心脏手术，ASA分级II-III且年龄大于或等于18岁；（2）术前心超检查左室射血分数（LVEF>35%）；（3）愿意签署知情同意书并完成所有研究内容。

Inclusion criteria

(1) Mitral valve replacement or plastic heart surgery under elective cardiopulmonary bypass in our hospital with ASA grade II-III and age greater than or equal to 18 years; (2) Left ventricular ejection fraction (LVEF>35%); (3) Willing to sign informed consent and complete all research content.

排除标准：

(1)年龄小于等于18岁的患者；（2）颈动脉狭窄程度>50%或有明显斑块者；（3）二次开胸或急诊手术者；（4）术前心超提示主动脉瓣明显病变者；（5）糖尿病、冠心病、风湿活动期、急慢性炎症期、自身免疫性疾病等,且近期服用激素、ACEI制剂、降尿酸类药物。（6）术前肝功能child-pugh分级为B级或C级，血肌酐超过177 μmol/L；（7）拒绝参与者。

Exclusion criteria：

(1) Patients aged 18 years or less; (2) Degree of carotid artery stenosis >50% or have significant plaque; (3) Second thoracotomy or emergency surgery; (4) Preoperative echocardiography indicated obvious aortic valve lesions; (5) diabetes mellitus, coronary heart disease, active period of rheumatism, acute and chronic inflammatory period, autoimmune diseases, etc., and recently taking hormones, ACEI preparations, urico-lowering drugs. (6)Preoperative child-pugh classification of liver function was B or C, with serum creatinine exceeding 177 μmol/L; (7) Reject participants.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1、门诊数据管理：保存门诊病历副本，扫描保存到项目专用的数据存储计算机；每位入组患者填写CRF表格，保存原始CRF表格，同时建立电子文档。 2、住院数据管理：出院后打印保存住院病历，扫描保存到项目专用的数据存储计算机；每位入组患者填写CRF表格，保存原始CRF表格，同时建立电子文档。 3.所有入组患者将血液化验检查的报告复印件保存，扫描保存。 4、数据录入前，数据录入人员应了解观察表中各项目的内容及编码。数据库命名应规范、易懂、易查找，并保证其正确性、安全性和保密性。输入数据可以通过二次输入或校对输入。如在录入过程中发现问题或意外情况，应及时登记上报，以便迅速处理问题。数据录入完成后，应随机抽查一些观察表，了解录入质量，分析处理存在的问题。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Outpatient data management: a copy of the outpatient medical record is stored, scans are saved to the project’s dedicated data storage computer, and the original CRF forms are stored and electronically completed by each patient.2. In-hospital data management: The hospital records are printed and stored after discharge, scanned and stored on a project-specific data storage computer.3. All enrolled patients shall have copies of their blood tests documented and scanned for preservation.4. Prior to data entry, the data entry person should be aware of the items and codes in the watch list.The database nomenclature should be standardized, comprehensible, and searchable, and ensure its accuracy, safety, and confidentiality.The input data can be re-entered or proofreaded.If any problems or unexpected events were noted during the enrollment process, they should be reported promptly so that problems can be dealt with promptly.After data entry was completed, some observational questionnaires were randomly sorted to understand the quality of entry and to analyze problems.
