研究目的：

探讨单纯后路固定治疗不稳定寰椎骨折的临床疗效  

Objectives of Study：

To investigate the clinical effect of single posterior fixation on unstable atlas fracture

药物成份或治疗方案详述：

观察组：取枕颈部后正中一直切口，显露枕骨粗隆下至颈2节段，剥离显露寰椎后弓，保留后弓骨折表面的骨膜，完整的骨膜对寰椎有固定作用，选取直径2mm大小的磨钻头，在寰椎的双侧椎弓根或侧块制备钉道，如果因为骨折后弓游动，助手可以用巾钳固定，制备钉道后测量其深度，选取适当型号的寰椎后弓固定板，安放时不要锁定中间的锁钉，并将固定板拉伸，使其长度稍大于寰椎两侧固定点的长度，首先固定寰椎两侧的螺钉，固定时尽量通过固定板两侧球窝结构的螺孔将螺钉固定到一个向内夹角的位置，这样可以通过加压固定板使分离的侧块复位，术区放置引流管一根，冲洗并关闭切口；对照组：显露枕骨粗隆下至颈3节段，剥离显露寰枢椎，同样选取直径2mm磨钻头，应用磨钻在双侧寰椎椎弓根或者侧块制备钉道，同理制备枢椎椎弓根通道，应用钉棒系统行寰枢椎固定，处理寰椎后弓以及枢椎椎板、棘突骨皮质，用取下的自体髂骨行寰枢椎融合。 

Description for medicine or protocol of treatment in detail：

Observation group: take occipital neck posterior median incision, expose occipital bone bulges down to neck 2 segments, peel exposed atlantoaxial arch, retain the periosteum on the surface of posterior arch fracture, complete periosteum has fixation effect on atlas, select grinding bit of diameter 2 mm, prepare the pedicle or lateral mass of atlantoaxial vertebra, if because of fracture arch, assistant can fix with towel forceps, measure its depth after preparing pedicle, select appropriate type of posterior atlantoaxial pedicle fixation plate, do not lock the length. As far as possible through the fixed plate on both sides of the ball hole of the ball socket structure on both sides of the plate as far as possible, so that the separated side block can be reset by pressurizing the plate, and a drainage tube is placed in the operation area to flush and close the incision;Control group: exposed occipital bone to cervical 3 segments, peeling exposed atlantoaxial vertebrae, also selected diameter 2 mm grinding bit, using grinding drill in bilateral atlantoaxial pedicle or lateral block to prepare the pedicle, the same preparation of the pivot pedicle channel, the application of nail rod system atlantoaxial fixation, treatment of the atlantoaxial arch as well as the pedicle, spinous cortex, with the removed autogenous iliac spine fusion. 

纳入标准：

(1)单纯寰椎骨折合并横韧带断裂；
(2)横韧带完整的前1/2Jefferson骨 折、前3/4Jefferson骨折、后3/4Jefferson骨折、半环骨折和经典 Jefferson骨折

Inclusion criteria

(1) simple atlantoaxial fracture with transverse ligament fracture;
(2) intact anterior 1/2 Jefferson fracture, anterior 3/4 Jefferson fracture, posterior 3/4 Jefferson fracture, semicircular fracture and classical Jefferson fracture

排除标准：

(1)横韧带完整 的前 1/4、后1/4、后1/2Jefferson骨 折；
(2)既往有上颈椎手术病史；
(3)伴有上颈椎结核、感染、肿瘤病史；
(4)伴有转移性肿瘤或血液系统肿瘤；
(5)其他影响骨代谢的疾病或药物服用者；
(6)枕颈部先天发育异常

Exclusion criteria：

(1)anterior 1/4, posterior 1/4, posterior 1/2 Jefferson fracture of complete transverse ligament;
(2) previous history of upper cervical surgery;
(3) history of upper cervical tuberculosis, infection, tumor;
(4) history of metastatic tumor or blood system tumor;
(5) other diseases affecting bone metabolism or drug users;
(6) congenital dysplasia of occipital neck.
