探索伴有呼吸窘迫的下呼吸道感染患儿中，使用 HFNC 呼吸支持治疗失败的临床预测指标。  

Objectives of Study：

To explore the clinical predictors of treatment failure with HFNC respiratory support in children with lower respiratory tract infections associated with respiratory distress.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 年龄 29 天-18 岁的男性或女性患儿； 2) 2024年3月至 2025 年2月，在上海儿童医学中心呼吸科住院治疗； 3) 临床诊断符合下呼吸道感染包括支气管炎、毛细支气管炎或肺炎； 4) 住院期间出现急性呼吸窘迫（急性呼吸窘迫定义为呼吸急促（< 2 个月：≥ 60次/min； 2-12 个月：≥ 50 次/min；1-5 岁：≥ 40 次/min; ≥ 5 岁：≥ 30次/min）伴有辅助呼吸肌运动包括三凹征阳性），使用鼻导管吸氧（2L/min）或面罩吸氧（5L/min），仍然存在呼吸窘迫的表现，经过呼吸专科主治以上医师认为需要使用 HFNC 治疗者； 5) 经知情同意进行本研究，所有受试儿童由法定监护人知情同意（8 岁以上儿童还需由本人知情同意）。

Inclusion criteria

1) Male or female children aged 29 days to 18 years; 2) Hospitalized in the Respiratory Department of Shanghai Children's Medical Centre from March 2024 to February 2025; 3) Clinical diagnosis of lower respiratory tract infection including bronchitis, bronchiolitis or pneumonia; 4) Acute respiratory distress (acute respiratory distress is defined as shortness of breath (< 2 months: ≥ 60 breaths/min; 2-12 months: ≥ 50 breaths/min; 1-5 years: ≥ 40 breaths/min; ≥ 5 years: ≥ 30 breaths/min) with assisted respiratory muscle movements, including a positive triple-concave sign), with oxygen administered through nasal cannula (2 L/min) or mask (5 L/min), and still having respiratory distress, and the patients were considered to be in need of HFNC treatment by a physician of respiratory speciality or above; 5) Informed consent for the study was obtained from the legal guardians of the children (children over 8 years of age were also required to give their informed consent).

排除标准：

1) 出现心脏或呼吸骤停； 2) Glasgow 评分<12 分； 3) 血液动力学不稳定，出现休克； 4) 呼吸性酸中毒（血气分析 pH<7.3 同时合并二氧化碳分压>50mmHg）； 5) 影像学检查提示存在气胸、气道异物征象； 6) 患儿无法配合 HFNC 治疗； 7) 研究者认为受试者有潜在风险或有干扰受试者的任何其他因素，不适宜参加本试验者。

Exclusion criteria：

1) Cardiac or respiratory arrest; 2) Glasgow score of <12; 3) Hemodynamic instability with shock; 4) Respiratory acidosis (blood gas analysis pH < 7.3 combined with a partial pressure of carbon dioxide > 50 mmHg); 5) Imaging suggests the presence of pneumothorax and airway foreign body signs; 6) The child is unable to co-operate with HFNC treatment; 7) The investigator considers the subject to be potentially at risk or has any other factor that interferes with the subject's ability to participate in the trial.
