研究目的: |
移植后糖尿病(post-transplant diabetes?mellitus,PTDM)是移植后常见的内分泌代谢紊乱疾病,发病率较高,使移植物相关并发症及心血管疾病、感染等风险增加,危害患者的生存质量及生存时间,降低受者生存率。糖尿病肾病发病机制包括糖代谢紊乱、胰岛功能渐进性损害、胰岛素抵抗,此外还涉及免疫调控机制等,其中STING信号通路在调节自身先天免疫功能是当今糖尿病领域的研究热点。SGLT-2抑制剂通过抑制肾近端小管葡萄糖重吸收、促进尿糖排泄而降低血糖,同时具有减重、降低血压、减少尿蛋白排泄等心血管及肾脏保护作用。国内文献尚未见针对肾移植后糖尿病联合应用SGLT-2抑制剂治疗的报道。本课题旨在对肾移植后糖尿病在常规降糖方案治疗基础上血糖仍然控制欠佳的患者,联合应用SGLT-2抑制剂达格列净,以期更好地控制患者血糖,减少糖尿病相关急慢性并发症,并随访观察达格列净对肾脏是否有保护作用,探讨达格列净肾功能的保护作用是否与cGAS-STING信号通路相关。 |

Objectives of Study: |
post-transplant diabetes mellitus (PTDM) is a common endocrine and metabolic disorder after transplantation, with a high incidence, which increases the risk of graft-related complications, cardiovascular diseases and infections, endangering the quality and survival time of patients, and reducing the survival rate of recipients. The pathogenesis of diabetic nephropathy includes disorders of glucose metabolism, progressive damage of islet function and insulin resistance, in addition to immune regulation mechanisms, among which STING signaling pathway in regulating innate immune function is a research hotspot in the field of diabetes. SGLT-2 inhibitors reduce blood sugar by inhibiting glucose reabsorption in the proximal renal tubules and promoting urinary glucose excretion, and have cardiovascular and renal protective effects such as weight loss, blood pressure reduction, and urinary protein excretion. There have been no reports in domestic literature on the treatment of post-renal transplantation diabetes with SGLT-2 inhibitors. The purpose of this study is to combine the SGLT-2 inhibitor dagaglizin in patients with diabetes after kidney transplantation who still have poor blood glucose control on the basis of conventional hypoglycemic treatment, so as to better control blood glucose in patients and reduce diabetes-related acute and chronic complications, and follow up to observe whether daglizin has a protective effect on the kidney. To investigate whether the protective effect of dagliar net renal function is related to cGAS-STING signaling pathway. |

(1)年龄在18~75周岁之间;(2)符合肾移植后糖尿病诊断标准;(3)8.0%≤HbA1c≤12.0%;(4)同意参加本研究并签署知情同意书者。 |

排除标准: |
(1) 合并1型糖尿病、2型糖尿病或其他原因及其他特殊类型糖尿病;(2)估算的肾小球滤过率<45mL/min/1.73m2;(3)泌尿生殖道感染者;(4)合并有严重心、肺、肝功能不全及恶性肿瘤患者;(5)患严重精神疾病等疾病无法配合或拒绝参加研究者;(6)对本研究使用药物过敏者;(7)妊娠期或哺乳期患者;(8)合并糖尿病严重并发症如糖尿病酮症酸中毒等或合并严重感染者。 |

Exclusion criteria: |
(1) combined with type 1 diabetes, type 2 diabetes or other causes and other special types of diabetes; (2) The estimated glomerular filtration rate was <45mL/min/1.73m2; (3) urogenital tract infection; (4) Patients with severe heart, lung, liver dysfunction and malignant tumors; (5) Unable to cooperate with or refuse to participate in the study due to serious mental illness; (6) Patients who are allergic to drugs used in this study; (7) Pregnant or lactating patients; (8) Severe complications of diabetes such as diabetic ketoacidosis or severe infections. |
