研究目的：

胃癌是临床常见的消化道恶性肿瘤，2020年全球胃发病例数达到108.9万例，胃癌已成为全球第五大癌症和第四大癌症死亡原因，严重威胁人类健康。目前外科手术仍是治疗胃癌的主要治疗手段。但是由于饮食摄入不足，大多数晚期胃癌患者营养不良。即使在胃癌早期，手术也会降低胃的消化能力，从而减少食物摄入，导致体重显著下降。因此胃癌病人常合并营养不良，营养不良是个恶性循环，营养治疗对胃癌患者是不可或缺的。有研究表明胃癌手术前后的肠道菌群发生变化，但是胃癌合并营养不良患者的菌群变化尚不清楚。此外，胃癌术后营养支持对肠道菌群的影响尚不清楚，尤其是术后早期肠内营养。 基于以上研究背景，本研究旨在从肠道菌群角度，招募胃癌合并营养不良患者，按是否术后早期肠内营养分组，通过16S rRNA测序分析肠道菌群，探索肠道菌群多样性的围术期变化及术后早期肠内营养对菌群的影响，并与健康受试者对照，以期阐明术后早期肠内营养的胃癌合并营养不良患者的菌群变化，为胃癌术后营养支持提供新的思路。  

Objectives of Study：

Gastric cancer is a common malignant tumor of the digestive tract in clinical practice. In 2020, the global number of gastric cases reached 1.089 million, and gastric cancer has become the fifth largest cancer and fourth largest cause of cancer death worldwide, posing a serious threat to human health. At present, surgery is still the main treatment method for gastric cancer. However, due to insufficient dietary intake, most patients with advanced gastric cancer suffer from malnutrition. Even in the early stages of gastric cancer, surgery can reduce the digestive capacity of the stomach, thereby reducing food intake and leading to significant weight loss. Therefore, gastric cancer patients often suffer from malnutrition, which is a vicious cycle, and nutritional treatment is indispensable for gastric cancer patients. There are studies indicating changes in gut microbiota before and after gastric cancer surgery, but the changes in microbiota in patients with gastric cancer and malnutrition are still unclear. In addition, the impact of postoperative nutritional support on gut microbiota in gastric cancer is still unclear, especially early postoperative enteral nutrition. Based on the above research background, this study aims to recruit patients with gastric cancer and malnutrition from the perspective of gut microbiota. They are divided into groups based on whether early postoperative enteral nutrition is necessary. 16S rRNA sequencing is used to analyze the gut microbiota, explore the perioperative changes in gut microbiota diversity, and the impact of early postoperative enteral nutrition on the microbiota. The aim is to clarify the changes in gut microbiota in gastric cancer and malnutrition patients with early postoperative enteral nutrition, and to compare with healthy subjects, Provide new ideas for postoperative nutritional support for gastric cancer.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.术前经胃镜取组织标本行病理学检查证实为胃癌，CT检查示未见远处器官转移，拟行腹腔镜胃癌根治术； 2.术前未行新辅助治疗； 3.年龄超过18岁； 4.入院后参考GLIM标准采用营养风险筛查及营养评估为营养不良。

Inclusion criteria

1. Preoperative pathological examination confirmed the diagnosis of gastric cancer by taking tissue samples through gastroscopy, and CT examination showed no distant organ metastasis. Laparoscopic radical gastrectomy for gastric cancer is proposed; 2. No neoadjuvant treatment was performed before surgery; 3. Over 18 years old; 4. After admission, nutritional risk screening and nutritional assessment were conducted according to GLIM standards to determine malnutrition.

排除标准：

1.术前伴有胃排空障碍、幽门梗阻等； 2.非恶性术后病理类型；残胃癌； 3.腹腔镜中转、肿瘤播种转移、姑息性手术和资料不全者； 4.合并精神障碍等不能配合治疗的疾病。

Exclusion criteria：

1. Preoperative complications include gastric emptying disorders, pyloric obstruction, etc; 2. Non malignant postoperative pathological types; Residual gastric cancer; 3. Laparoscopic transfer, tumor seeding and metastasis, palliative surgery, and incomplete data; 4. Diseases such as comorbid mental disorders that cannot be treated in combination.

术后麻醉苏醒后即可嘱病人饮一口清水，术后第1天可按每1小时30ml的速度、总量300ml的方案饮水或经空肠管输注清水，术后第2天可按每1小时60ml的速度、总量600ml的方案饮水或经空肠管输注清水，术后第3天可按每1小时90ml的速度、总量900ml的方案饮水或经空肠管输注清水

After awakening from postoperative anesthesia, the patient can be instructed to drink a mouthful of clean water. On the first day after surgery, the patient can drink water at a rate of 30ml per hour and a total of 300ml, or be infused with clean water through a jejunal tube. On the second day after surgery, the patient can drink water at a rate of 60ml per hour and a total of 600ml, or be infused with clean water through a jejunal tube.
