研究目的：

1、明确急性缺血性脑卒中患者肠道菌群及自主神经功能的变化特点。 2、探究益生菌Probio-M8对卒中后认知障碍的治疗效果和作用机制以及益生菌Probio-M8、自主神经系统功能、卒中后认知功能障碍之间的关系。  

Objectives of Study：

1. To clarify the change characteristics of intestinal flora and autonomic nervous function in patients with acute ischemic stroke. 2. To explore the therapeutic effect and mechanism of probiotics Probio-M8 on post-stroke cognitive impairment and the relationship between probiotics Probio-M8, autonomic nervous system function and post-stroke cognitive impairment.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1: 50岁≤年龄≤80岁； 2: 首次发病1周内由计算机断层扫描或磁共振成像诊断为缺血性卒中（NIHSS≤18）并且为幕上梗死，符合2018版中国急性缺血性脑卒中诊治指南； 3: 存在脑卒中急性期认知功能障碍（MoCA评分＜25）并且非神经系统变性疾病（IQCODE评分＜3.19）； 4: 患者意识清楚，能够自己或者在家人的帮助下配合完成检查操作、问卷调查、病史采集等； 5: 同意参与本研究，签署知情同意书。

Inclusion criteria

1: 50 years≤Age≤80 years; 2: Ischemic stroke (NIHSS≤18) and supratentorial infarction were diagnosed by computed tomography or magnetic resonance imaging within 1 week of the first onset, which was in line with the 2018 Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke 3: Patients with acute stroke cognitive impairment (MoCA score < 25) and non-neurodegenerative disease (IQCODE score < 3.19); 4: The patient has clear consciousness and can cooperate with himself or with the help of his family to complete the examination operation, questionnaire survey, history collection, etc; 5: Agree to participate in this study and sign the informed consent.

排除标准：

1: 非首次缺血性卒中、非幕上梗死、出血性卒中、外伤或创伤性手术引起的卒中； 2: 卒中前进展性认知功能障碍（IQCODE评分≥3.3）； 3: 既往颅脑外伤、癫痫、重度抑郁或焦虑等可能影响认知功能的疾病； 4: 既往存在非窦性心律 (如心房颤动、心房扑动、节奏性心律或其他心律失常)、心肌梗死、心力衰竭或安装心脏起搏器等可能影响自主神经系统的心脏疾病以及严重肺、肝、肾疾病或恶性肿瘤等其他可能影响实验结果的严重共患病； 5: 无法完成神经心理测试评估，如严重失语、视听及肢体功能障碍或意识障碍以致影响书面及言语表达者； 6: 需长期服用免疫抑制剂或入组前1月内服用抗生素及随访期间服用抗生素的患者或随访期间停服或随意调整受试样品者或近3月内服用过地高辛、α受体阻滞剂、β受体阻滞剂、抗心律失常药物、支气管扩张剂和糖皮质激素等可能影响自主神经功能系统的药物； 7: 随访期间同时服用促排便药物，如聚乙二醇电解质、酚酞片、番泻叶等患者； 8: 患有慢性消化系统疾病或肿瘤，如消化道溃疡、炎症性肠炎、严重肝病等患者； 9: 对受试样品过敏或出现明显消化道症状如不能耐受者； 10: 随访期间饮食习惯不规律，有较大变动的患者。

Exclusion criteria：

1: Stroke not due to first ischemic stroke, supratentorial infarction, hemorrhagic stroke, trauma, or traumatic surgery 2: Progressive cognitive impairment after stroke (IQCODE score ≥3.3); 3: Pre-existing conditions such as traumatic brain injury, epilepsy, and severe depression or anxiety may affect cognitive function; 4: Patients had a history of heart diseases such as non-sinus rhythm (atrial fibrillation, atrial flutter, rhythmic rhythm or other arrhythmia), myocardial infarction, heart failure or cardiac pacemaker, which may affect the autonomic nervous system, and other serious comorbidities such as severe lung, liver, kidney diseases or malignant tumors, which may affect the experimental results. 5: People who are unable to complete neuropsychological assessment, such as severe aphasia, visual and physical impairment, or impairment of consciousness that affects written and verbal expression. 6: Patients who need to take immunosuppressants or antibiotics within 1 month before enrollment and during follow-up, or who stop taking or arbitrarily adjust the sample during follow-up, or have taken digoxin, alpha-blockers, beta-blockers, antiarrhythmic drugs, bronchodilators, glucocorticoids and other drugs that may affect the autonomic nervous system in the last 3 months. 7: During the follow-up period, patients also took defecation promoting drugs, such as polyethylene glycol electrolyte, phenolphthalein tablets, senna leaf, etc. 8: Patients with chronic digestive diseases or tumors, such as gastrointestinal ulcer, inflammatory enteritis, severe liver disease, etc. 9: People who are allergic to the tested samples or show obvious gastrointestinal symptoms, such as being unable to tolerate them. 10: people who have irregular dietary habits and large changes during the follow-up period.
