研究目的：

人口老龄化是世界各国面临的一个共同的难题，我国人口基数大、老年人群增长速度快，预计在2035年全国60岁以上老年人口约4亿，占总全国人口的30%以上。老年人群的健康问题已成为国家关注的重点问题之一。 国家基本公共卫生服务项目是我国政府针对当前城乡居民存在的主要健康问题面向全体居民免费提供的最基本的公共卫生服务，包括老年人体检项目。老年体检项目主要包括对老年人的生活方式和健康状况评估、体格检查（一般状况、五官检查、内外科检查等）、辅助检查（空腹血糖、血常规、尿常规、肝功能、肾功能、血脂、心电图、腹部彩超、胸片等）、中医体质辨识、家庭医生签约和健康评价与指导等。但是目前基于老年体检项目对老年人的健康状况进行系统分析仍较为缺乏。前期本项目研究团队依据上海市社区卫生服务中心的体检项目初步了解到老年人健康状况，但对于系统了解老年人的健康状况尤其是影响因素还缺少相关信息的收集。因此，本项目拟在老年人健康体检项目的基础上，收集其他影响老年人健康状况因素的信息，进而系统分析上海市老年人健康状况及影响因素，为老年人群疾病防控提供科学依据。  

Objectives of Study：

The aging population is a common problem faced by countries around the world. China has a large population base and a fast growth rate of the elderly population. It is expected that by 2035, the national population of elderly people aged 60 and above will be about 400 million, accounting for over 30% of the total national population. The health issues of the elderly population have become one of the key issues of national concern.The national basic public health service project is the most basic public health service that our government provides free to all residents in view of the main health problems existing in urban and rural residents at present, including the physical examination project for the elderly. Physical examination items for the elderly mainly include assessment of lifestyle and health status of the elderly, physical examination (general condition, facial examination, internal and external examination, etc.), auxiliary examination (Glucose test, Fasting blood sugar, blood routine examination, urine routine examination, liver function, kidney function, blood fat, electrocardiogram, abdominal color ultrasound, chest film, etc.), TCM constitution identification, family doctor signing, Health assessment and guidance, etc. However, there is still a lack of systematic analysis of the health status of the elderly based on elderly physical examination projects. In the early stage, the research team of this project had a preliminary understanding of the health status of the elderly based on the physical examination program of the Shanghai Community Health Service Center. However, there is still a lack of relevant information collection to systematically understand the health status of the elderly, especially the influencing factors. Therefore, this project aims to collect information on other factors that affect the health status of the elderly based on the elderly health examination project, and then systematically analyze the health status and influencing factors of the elderly in Shanghai, providing scientific basis for disease prevention and control among the elderly population.

(1)上海市社区卫生服务中心辖区内所有老年人,65岁以上 (2)受试者自愿加入本研究,签署知情同意书,依从性好,配合随访

Inclusion criteria

(1) All elderly people aged 65 and above within the jurisdiction of Shanghai Community Health Service Center (2) The subjects voluntarily joined this study, signed an informed consent form, had good compliance, and cooperated with follow-up
