研究目的：

脑卒中作为世界三大致死性疾病之一，其死亡率及致残率极高。吞咽障碍是脑卒中后常见的并发症之一,卒中后吞咽障碍患者发病率在 70%左右，因脑干中疑核和孤束核等控制和调节吞咽反射的中枢网状系统,因此,脑干卒中后病人发生吞咽障碍的发生率较其他部位梗死病人更高,且病人极易因误吸导致吸入性肺炎等不良后果。本项目从具身虚拟镜像视觉反馈的角度出发，运用综合的测评方法和个性化运动处方对 72例 21至 70岁四川地区脑干卒中后吞咽障碍患者的吞咽功能和情绪状况进行评估治疗，旨在提高脑干卒中后吞咽障碍患者的康复效果，并推广一套具有广泛实用性的脑干卒中后吞咽障碍患者的治疗处方，以期为进行个体化康复治疗与情绪干预提供方案.  

Objectives of Study：

Stroke, as one of the three deadly diseases in the world, has extremely high mortality and disability rates. Dysphagia is one of the common complications after stroke. The incidence rate of dysphagia patients after stroke is about 70%. Because the central reticular system in the brain stem, such as nucleus ambiguus and nucleus tractus solitarius, controls and regulates the swallowing reflex, the incidence of dysphagia in patients after brain stem stroke is higher than that in patients with infarction in other parts, and patients are very prone to inhalation pneumonia and other adverse consequences due to aspiration. From the perspective of embodied virtual mirror visual feedback, this project evaluates and treats the swallowing function and emotional status of 72 patients with swallowing disorders after brainstem stroke in Sichuan, aged 21 to 70, using comprehensive evaluation methods and personalized exercise prescriptions. The aim is to improve the rehabilitation effect of swallowing disorders after brainstem stroke and promote a set of widely practical treatment prescriptions for brainstem stroke swallowing disorders, To provide a plan for personalized rehabilitation treatment and emotional intervention .

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①脑干卒中成年患者，均经头颅MRI扫描证实； ②吞咽造影显示有咽期吞咽障碍； ③无鼻腔或咽部器质性梗阻等检查的禁忌证； ④年龄40—70岁四川地区居民； ⑤病程3个月 一1年； ⑥能够遵照执行治疗师的要求并坚持治疗； ⑦患者或家属均签署知情同意书，符合医学伦理学要求。

Inclusion criteria

① Adult patients with brainstem stroke were confirmed by cranial MRI scans; ② Swallowing imaging shows swallowing disorders during the swallowing phase; ③ No contraindications for examination of nasal or pharyngeal organic obstruction; ④ Residents of Sichuan region aged 40-70; ⑤ The course of the disease is 3 months to 1 year; ⑥ Able to follow the requirements of the executing therapist and adhere to treatment; ⑦ Both the patient and their family members sign an informed consent form, which meets the requirements of medical ethics.

排除标准：

①伴有心肺功能代偿不全、传染性疾病、发热、身体极度衰弱和各种出血倾向者，近一周癫痫大发作者； ②多发脑梗死病灶，皮质、小脑、丘脑等部位病灶； ③既往有脑出血、蛛网膜下 腔出血、脑肿瘤、脑外伤及其他神经精神系统病史； ④既往鼻咽癌放疗、头颈癌病变患者； ⑤既往有吞咽困难病史；合并有认知功能障碍、失语； ⑥有严重心肺肾并发症； ⑦不能配合检查与治疗者。

Exclusion criteria：

① Individuals with decompensated cardiovascular function, infectious diseases, fever, extreme physical weakness, and various bleeding tendencies, who have experienced a major outbreak of epilepsy in the past week; ② Multiple cerebral infarction lesions, including lesions in the cortex, cerebellum, and thalamus; ③ Previous history of cerebral hemorrhage, subarachnoid hemorrhage, brain tumors, traumatic brain injury, and other neurological and psychiatric disorders; ④ Patients with previous nasopharyngeal carcinoma radiotherapy and head and neck cancer lesions; ⑤ Previous history of swallowing difficulties; Combined with cognitive impairment and aphasia; ⑥ Having serious cardiovascular, pulmonary, and renal complications; ⑦ Those who cannot cooperate with the examination and treatment.
