研究目的：

结直肠癌在我国发病率位于恶性肿瘤发病率的第三位，死亡率位居第五位。由于目前结直肠癌早期诊断、早期治疗的实施及治疗水平的不断提高，结直肠癌患者生存人群数量逐渐增大。如何科学合理地管理日趋庞大的结直肠癌幸存者并延长其健康生存时间亟待研究。结直肠癌疾病本身引起的生理及病理性改变及其治疗方法将会引起患者身体功能、生理功能及精神状态的改变，从而导致患者生命质量发生变化。研究显示结直肠癌患者自评生命质量是其生存的独立预后因素之一，同时这些自评生命质量可以帮助临床医生做出更为科学合理的治疗决策。目前使用的量表绝大多数源于欧美人群，由于文化背景的不同，很多内容并不适用于中国癌症患者。因此，目前亟需一套基于中国国情和文化背景，适用于中国结直肠癌患者的自评生命质量量表。基于上述背景，课题组将开展多中心结直肠癌患者前瞻性队列研究，研发中国结直肠癌患者自评量表，为建立中国结直肠癌患者院后健康管理指南提供科学依据。  

Objectives of Study：

The incidence and mortality rates of colorectal cancer rank third and fifth,respectively, among all malignant tumors in China. Due to the recent implementation of early diagnosis and early treatment of colorectal cancer and the gradual improvement of treatment effect, the number of cancer survivors is continuous increasing. There is an urgent need to investigate how to manage the growing number of colorectal cancer survivors and prolong their healthy survival time in a scientific and reasonable manner. The physiological and pathological changes caused by the disease and cancer-related treatments will alter physical function, physiological function and mental state of patients, thus leading to changes in the patients' quality of life. Previous research suggested that self-evaluated quality of life by colorectal cancer patients was one of the independent prognostic factors for survival, and these self-evaluated quality of life could help physicians make more scientific and reasonable treatment decisions. Most of the currently used scales for quality of life originate from the European and American populations. Due to different cultural backgrounds, these scales are not perfectly applicable to the Chinese cancer patients. Therefore, there is an urgent need to develop a self-rating scale of quality of life based on China's national conditions and cultural background, and make it suitable for Chinese colorectal cancer patients. Based on the above reasons, we will conduct a multi-center prospective cohort study of colorectal cancer patients to develop a self-rating scale of quality of life and provide a scientific basis for establishing post-hospital health management guidelines for Chinese colorectal cancer patients.

纳入标准：

结直肠癌诊断明确；无其他恶性肿瘤；知情同意且自愿参加本课题；参与者具有完全认知和行为能力，能独立完成量表。

Inclusion criteria

Newly diagnosed colorectal cancer; no other malignant tumors; informed consent and voluntary participation; full cognitive behavioral ability and able to complete the scale independently.

排除标准：

既往有精神、神经系统疾病或有其他重大疾病史；语言沟通障碍者。

Exclusion criteria：

With history of mental, neurological, or other major diseases; with communication disorders.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

患者入组后，由每个研究实施点的研究人员录入患者的就诊资料和病例信息，并上传患者诊疗信息（包括病历首页、入院记录及出院记录）至数据后台。自评生命质量量表将采用微信小程序问卷的方式按照时间节点推送至患者微信，患者的作答记录将以电子版的形式直接传送及保存在数据后台。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Researchers at participant hospitals will enter the medical record and treatment information for each patient after recruitment, and upload these data (including copies of the home page of medical record, as well as admission and discharge record) to the central database. The self-rating scale of quality of life will be electronically pushed to the patients' WeChat at each planned follow-up time point in the form of WeChat mini-program, and the patient's answer record will be directly transmitted in electronic form and saved in the central database.
