研究目的：

构建基于Van Houtven护理框架的失能老人照护者照护技能干预方案,观察其实践效果。为照护者提供实践指导，学习相关照护技能，以提升照护能力，促进失能老人健康维护降低照顾者照顾负担。丰富我国人口深度老龄化国情下的失能老年人照护体系，开辟我国失能老人照护的新方法，尤其是社区卫生服务站的广泛推广应用  

Objectives of Study：

The nursing skills intervention scheme of disabled elderly caregivers based on Van Houtven nursing framework was constructed and its practical effect was observed. To provide practical guidance and learn relevant care skills for caregivers, so as to improve the care ability, promote the health maintenance of disabled elderly people and reduce the care burden of caregivers. To enrich the care system of the disabled elderly under the condition of deep aging in our country, and develop a new method of care for the disabled elderly, especially the widespread application of community health service stations

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.失能老人纳入标准： ①年龄≥60岁； ②根据巴塞尔指数评估住院前为中、重度失能者； ③被照护时间≥3个月； ④意识清晰，有一定的理解、配合能力； ⑤失能老人知情同意，自愿接受本次实验，并自愿签署知情同意书。 2.家庭照护者纳入标准： ①年龄≥18岁； ②为失能老人的子女（包括儿媳与女婿）； ③照护失能老人时间≥1个月（照护者＞１名时，选择每天照护时间最长者）； ④无沟通交流障碍； ⑤对本实验知情，同意配合，并自愿签署知情同意书。

Inclusion criteria

1. Inclusion criteria for disabled elderly: ① Age ≥60 years old; ② Moderate to severe disability prior to hospitalization was assessed according to the Basel Index; ③ The duration of care ≥3 months; ④ Clear consciousness, have a certain understanding, coordination ability; ⑤ Disabled elderly people gave informed consent, voluntarily accepted this experiment, and voluntarily signed informed consent. 2. Inclusion criteria for family caregivers: ① Age ≥18 years old; (2) the children of the disabled elderly (including daughter-in-law and son-in-law); (3) Care time for disabled elderly ≥1 month (when the caregiver > 1, choose the longest daily care time); ④ no communication barriers; ⑤ Know about this experiment, agree to cooperate, and voluntarily sign the informed consent.

排除标准：

1.失能老人纳入标准： 处于生命终末期的老年人 2.家庭照护者纳入标准： 曾接受过相关照护培训的照护者

随机方法（请说明由何人用什么方法产生随机序列）：

团队的统计学人员使用计算机随机生成的随机数字来进行随机化,对选取的患者进行随机化分组。采用完全随机设计的方法，对选取的60例患者进行随机化分组，将每位患者进行入院顺序性编号，即1-60号，应用Excel中“函数-Rand”求得随机数，将此列随机数进行排序，1-30号对应的患者进入对照组，31-60号对应的患者进入实验组。

An independent statistician will assign participants using the computer-generated random sequence, and the process is blind to other investigators. A completely random design method was adopted to randomly group 60 patients, and each patient was assigned a sequential admission number, that is, number 1-60. Random numbers were obtained by using "function-RAND" in Excel, and the random numbers in this column were sorted. Patients corresponding to numbers 1-30 entered the control group, and patients corresponding to numbers 31-60 entered the experimental group.
