通过本课题的研究,对比定制式3D打印骨模型在甲状腺相关眼病外侧眼眶减压术中的应用和传统手术组,探讨各分组患者病情改善的变化,并对患者的手术持续时间、眼球突出度、眼球运动、临床活动评分、复视情况以及术后并发症进行评估。探讨定制式3D打印骨模型在甲状腺相关眼病外侧眼眶减压术中的应用疗效。  

Objectives of Study:

Through the research of this project, the application of customized 3D printed bone models in lateral orbital decompression surgery for thyroid related eye diseases was compared with traditional surgical groups, and the changes in the improvement of patients in each group were explored. The duration of surgery, eye protrusion, eye movement, clinical activity score, diplopia, and postoperative complications of patients were evaluated. Exploring the application and therapeutic effect of customized 3D printed bone models in lateral orbital decompression surgery for thyroid related eye diseases.

纳入标准:

极重度或中重度静止期拟行外侧眼眶减压手术的甲状腺相关眼病的患者。

Inclusion criteria

Patients with thyroid related eye disease who plan to undergo lateral orbital decompression surgery during extremely severe or moderate to severe stationary phase.

排除标准:

年龄小于18岁或超过65岁患者;合并全身有消耗性或免疫性基础疾病患者;担心手术风险,对手术治疗有顾忌的患者。

Exclusion criteria:

Patients under 18 years old or over 65 years old; Patients with systemic wasting or immunological underlying diseases; Patients who are concerned about surgical risks and have concerns about surgical treatment.

患者在术前均常规行眼眶部64排螺旋CT水平、冠状位扫描以及三维重建。选择的窗宽和窗位以能充分显示软组织嵌顿和骨组织缺损情况为准。扫描层厚为0.75mm,原始图片分辨率为512×512。将所得的医学数字图像和通迅(digital imaging and communication in medicine,DICOM)文件原始数据刻录至光盘。利用Minics16.0软件(比利时,Materialise公司)对光盘导入的DICOM数据选取合适的灰度值并进行二值化、区域增长以及去除漂浮像素等处理,进而实现三维模型重建。将重建后得到的眼眶骨折模型数据以立体光固化成型(stereo lithography,STL)文件的格式保存并导入到逆向工程软件Geomagic studio 12.0(美国,Geomagic公司)中。通过该软件将健侧眼眶模型镜像翻转至患侧以创建修复模型。运用套索工具将骨折区域精确选中,通过“抽壳”等命令设计出与解剖形态一致的骨缺损区域导板。将设计好的导板和眼眶骨折模型数据导入至医用快速成型机(黑焰医疗-450)进行三维打印。打印材料为液体光固化树脂,打印参数为: 激光功率260mW、支撑扫描速率1600 mm/s、填充扫描速率7000 mm/s、轮廓扫描速率6000mm/s。将打印出的模型和导板进行清洁和干燥处理。术中a组应用传统眼眶减压完成手术,b组借助3D模型辅助完成手术。

干预措施代码:

Intervention:

Patients underwent routine 64 slice spiral CT horizontal, coronal, and 3D reconstruction of the orbit before surgery. The selected window width and window position should fully display the situation of soft tissue entrapment and bone tissue defects. The scanning layer thickness is 0.75mm, and the original image resolution is 512 × 512. Burn the raw data of the obtained medical digital images and communication in medicine (DICOM) files onto a CD. Using Minics16.0 software (Materialise, Belgium), select appropriate grayscale values for the DICOM data imported from the CD and perform binarization, region growth, and removal of floating pixels to achieve 3D model reconstruction. Save the reconstructed orbital fracture model data in the format of Stereo Lithography (STL) file and import it into the reverse engineering software Geomagic Studio 12.0 (Geomagic Company, USA). Use this software to flip the image of the healthy eye socket model onto the affected side to create a repair model. Use lasso tools to accurately select the fracture area, and design a bone defect area guide plate that is consistent with the anatomical shape through commands such as "shell extraction". Import the designed guide plate and orbital fracture model data into a medical rapid prototyping machine (Black Flame Medical 450) for 3D printing. The printing material is liquid photocurable resin, and the printing parameters are: laser power 260mW, support scanning rate 1600 mm/s, filling scanning rate 7000 mm/s, and contour scanning rate 6000mm/s. Clean and dry the printed model and guide plate. During the surgery, Group A used traditional orbital decompression to complete the surgery, while Group B used 3D models to assist in the surgery.
