研究目的：

观察右美托咪定术前短期滴鼻是否可以改善心脏病患者睡眠障碍，从而降低术后谵妄。  

Objectives of Study：

To observe whether short-term preoperative nasal drops of dexmedetomidine can improve sleep disturbance in cardiac patients and thus reduce postoperative delirium.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.60-75岁；
2.冠心病拟行不停跳冠状动脉搭桥手术；
3.中度二尖瓣狭窄或关闭不全；
4.中度主动脉瓣狭窄或关闭不全；
5.主动脉根部手术；
6.近期睡眠障碍（匹兹堡睡眠质量指数≥8分）。

Inclusion criteria

1. Aged 60-75 years;
2. To perform Off-pump coronary artery bypass grafting surgery;
3. Moderate mitral stenosis or insufficiency;
4. Moderate aortic stenosis or insufficiency;
5. Aortic root lesions to be operated;
6. Preoperative sleep disturbance (PSQI >= 8).

排除标准：

1.基线最低精神状态检查（MMSE）得分≤20分；
2.不同意或无法同意参加研究和/或无法进行说话；
3.听力障碍；
4.睡眠呼吸暂停综合征；
5.入院后射血分数小于等于50%。

Exclusion criteria：

1. Minimum mental state examination (MMSE) score <= 20;
2. Disagreement or inability to agree to participate in the study and/or to speak;
3. Patients with hearing disorders ;
4. Sleep apnea syndrome;
5. Ejection fraction <= 50% after admission.

独立麻醉医生术前一天访视患者并解释研究目的，签署研究知情同意书。通过计算机生成的随机数字将受试者按1：1分为A组、B组，将组分配密封在顺序编号的不透明信封中，在患者签许知情同意书后将其打开，研究者根据小组分配的情况准备试验方案，

The independent anesthesiologist visits the patient the day before surgery to explain the purpose of the study and sign an informed consent form for the study. Using computer generated random numbers, the subjects were divided into groups a and B at the rate of 1:1.
