研究目的：

通过观察由麻醉医生主导的认知行为疗法、麻醉睡眠平衡术、星状神经节阻滞、经颅磁刺激等构成的多模式睡眠治疗方案治疗顽固性失眠的临床有效性和安全性，对顽固性失眠的治疗方案进行改进和优化，为顽固性失眠患者提供更加有效可行的治疗依据。  

Objectives of Study：

To observe the clinical efficacy and safety of multimodal sleep therapy consisting of cognitive behavioral therapy, anesthesia sleep balance surgery, stellate ganglion block and transcranial magnetic stimulation in the treatment of refractory insomnia, and to improve and optimize the treatment plan for refractory insomnia, so as to provide a more effective and feasible treatment basis for patients with refractory insomnia.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合上述西医及中医的失眠诊断标准； （2）发病年龄为18-65岁之间，男女不限； （3）病程在6个月及以上； （4）匹兹堡睡眠质量指数（PSQI）评分在7分以上，汉密顿抑郁量表评分小于24分，汉密顿焦虑量表评分小于21分； （5）从未服用过相关镇静安神类药物，或服用后无疗效且停药时间超过1个月； （6）识字、能自我认知并自主完成各种评分量表； （7）能够配合坚持本方案治疗，完成所有疗程，依从性良好。

Inclusion criteria

(1) meet the above-mentioned diagnostic criteria of western medicine and traditional Chinese medicine for insomnia; (2) The age of onset was between 18 and 65 years old, both male and female; (3) the course of disease was 6 months or more; (4) Pittsburgh Sleep Quality Index (PSQI) score was more than 7 points, Hamilton Depression Scale score was less than 24 points, and Hamilton Anxiety Scale score was less than 21 points. (5) Patients had never taken relevant sedative and tranquilizing drugs, or had no effect after taking them and the drug withdrawal time was more than 1 month; (6) able to read, self-recognize and complete various rating scales independently; (7) can cooperate and adhere to the treatment of this regimen, complete all courses of treatment, and have good compliance.

排除标准：

（1）不能排除由其他疾病，如精神类疾病、器质性疾病等引起的失眠患者； （2）特殊人群，如妊娠期、哺乳期女性； （3）合并有严重的心、脑、肝、肾等器官的病变； （4）同时进行其他治疗，可能会对本研究结果造成影响者； （5）对本研究相关药物过敏者； （6）存在神经阻滞禁忌症患者。

Exclusion criteria：

(1) Patients with insomnia caused by other diseases, such as mental diseases and organic diseases, cannot be excluded; (2) special population, such as pregnant and lactating women; (3) complicated with serious heart, brain, liver, kidney and other organs lesions; (4) patients receiving other treatments at the same time, which may affect the results of this study; (5) patients with drug allergy; (6) patients with contraindications to nerve block.
