研究目的：

使用非侵入性和低成本的功能性近红外光谱成像技术，观测原发性失眠症患者重复经颅磁刺激后前额叶皮层脑区激活和功能网络的变化。为经颅磁刺激治疗提供了基础，并将进一步提高其在临床实践中的应用。  

Objectives of Study：

Using non-invasive and low-cost functional near-infrared spectroscopy imaging technology, observe the changes in activation and functional network of prefrontal cortex brain regions in patients with primary insomnia after repeated transcranial magnetic stimulation. This provides a foundation for transcranial magnetic stimulation therapy and will further enhance its application in clinical practice.

药物成份或治疗方案详述：

招募原发性失眠症患者和健康受试者，分为实验组和对照组。所有受试者及家属在治疗前均被告知治疗可能的风险及疗效，并签署知情同意书。在刺激前统计受试者的基线数据。患者放松坐在治疗椅上，佩戴定位帽，测出患者的静息运动阈值以及确定右侧背外侧前额叶，通过Mag-TD经颅磁刺激仪对靶点进行单一刺激和长达2周磁刺激，频率1Hz、强度80%RMT、1500次脉冲(持续10s,间隔2s），每次治疗持续30min。刺激结束后，通过功能近红外仪器收集受试者前额叶脑区的HbO2和功能连接变化，以及量表评估患者症状的改善。 

Description for medicine or protocol of treatment in detail：

Recruit patients with primary insomnia and healthy subjects, and divide them into an experimental group and a control group. All subjects and their families were informed of the potential risks and efficacy of treatment before treatment, and signed an informed consent form. Calculate the baseline data of the subjects before stimulation. The patient relaxed and sat on the treatment chair, wore a positioning cap, measured the resting motion threshold of the subject, and determined the right dorsolateral prefrontal lobe. The target was subjected to single and 2-week magnetic stimulation using a Mag-TD transcranial magnetic stimulator, with a frequency of 1Hz, an intensity of 80% RMT, and 1500 pulses (lasting for 10 seconds, with an interval of 2 seconds). Each treatment lasted for 30 minutes. After stimulation, HbO2 and functional connectivity changes in the prefrontal cortex of the subjects were collected using a functional near-infrared instrument, and the improvement of patient symptoms was evaluated using a scale.  

纳入标准：

（1）原发性失眠症患者。 （2）患者都为右利手。 （3）年龄18-60岁。 （4）匹兹堡睡眠质量指数＞7分。 （5）每周出现睡眠障碍至少3次。 （6）病程≥3个月。 （7）认知功能良好，日常生活活动无障碍。

Inclusion criteria

(1) Patients with primary insomnia. (2) The patients are all right-handed. (3) Age 18-60 years old. (4) Pittsburgh Sleep Quality Index>7 points. (5) Sleep disorders occur at least 3 times a week. (6) The course of the disease is ≥ 3 months. (7) Good cognitive function and accessibility to daily activities.

排除标准：

（1）患者患有严重的心血管、肝、肾或造血系统疾病。 （2）患者失眠是由神经系统疾病（如中风、帕金森病）引起的。 （3）患者失眠由包括抑郁或焦虑在内的精神障碍引起。 （4）患者有睡眠呼吸暂停史。 （5）有癫痫病史或目前癫痫发作诊断尚未控制。 （6）有植入性心脏起搏器、除颤器以及其他体内植入型仪器。 （7）患者有经颅磁刺激禁忌症。 （8）患者在试验前两周服用了抗抑郁药物或抗精神病药物。

Exclusion criteria：

(1) The patient has severe cardiovascular, liver, kidney, or hematopoietic system diseases. (2) Insomnia in patients is caused by neurological diseases such as stroke and Parkinson's disease. (3) Patient insomnia is caused by mental disorders including depression or anxiety. (4) The patient has a history of sleep apnea. (5) Having a history of epilepsy or currently diagnosed with an uncontrollable seizure. (6) There are implanted pacemakers, defibrillators, and other implanted instruments in the body. (7) The patient has contraindications to transcranial magnetic stimulation. (8) The patient took antidepressants or antipsychotics two weeks before the trial.
