研究目的:

前交叉韧带(ACL)重建术后重返运动标准的制定是目前的难点,研究发现重建术后有脑神经重塑且存在个体差异。静息态功能磁共振(fMRI)的低频振幅(ALFF)及功能连接(FC)可探索不同人群神经功能的差异性,但尚未应用于 ACL 术后重返运动的研究。本项目拟运用 ALFF 算法,计算 ACL 术后重返运动者与不能重返者 BLOD 信号及 FC 的差异,以及探索默认网络(DMN)及感觉运动网络(SMN)的关系,并联合使用三维步态分析进行精准功能测试。本项目将从新的角度探索 ACL 术后功能恢复的中枢策略,为重返运动时机提供更合理的评估指标,旨在降低术后再撕裂的发生率  

Objectives of Study:

At present, it is difficult to establish the standard of return to exercise after anterior cruciate ligament (ACL) reconstruction Neuroremodeling was observed after operation and there were individual differences. Low-frequency amplitude of resting state functional magnetic resonance (fMRI) (ALFF) and Functional connectivity (FC) can explore the differences in neural function among different populations, but they have not yet been applied

Description for medicine or protocol of treatment in detail:

 

纳入标准:

病例组入选标准: 1)年龄≥18岁,且≤50岁,性别不限;2)影像学或者术中镜下明确前交叉韧带撕裂并由同一术者进行ACL重建手术且术后大于2年的患者;3)自愿参加脑功能磁共振检查4)患者知情同意,并签字获得知情同意书,过程符合GCP要求。

Inclusion criteria

Inclusion criteria of case group: 1) Age ≥18 years old, ≤50 years old, regardless of gender; 2) Patients with anterior cruciate ligament tear confirmed by imaging or intraoperative microscopy and ACL reconstruction surgery performed by the same surgeon more than 2 years after surgery; 4) The patient gave informed consent and signed the informed consent, which met the requirements of GCP.

排除标准:

1)既往前交叉韧带手术史;2)合并其他严重膝关节损伤(内侧副韧带断裂、后交叉韧带损伤、髌骨脱位、软骨III-IV 损伤等),其他严重影响运动功能的运动损伤者;3)具有其他严重的原发性心血管病变、肺脏疾病、内分泌及代谢性疾病或影响其生存的严重疾病者;4)严重的神经、精神疾病患者5)体内有金属、心脏及神经起搏器安装者。

Exclusion criteria:

1) History of anterior cruciate ligament surgery; 2) Patients with other serious knee joint injuries (medial collateral ligament rupture, posterior cruciate ligament injury, patella dislocation, cartilage III-IV injury, etc.) or other sports injuries that seriously affect motor function; 3) Have other serious primary cardiovascular diseases, lung diseases, endocrine and metabolic diseases or serious diseases affecting their survival; 4) Patients with severe neurological and mental diseases 5) patients with metal, heart and nerve pacemakers in the body.
