研究目的：

术后谵妄主要见于老年患者并且发展成为在老年患者手术后的主要并发症之一，其严重的影响患者术后生活质量。据报道，60 岁以上老年病人非心脏手术后 7 天 POD 的发生率为 25.8％，术后 3 个月 POD 发生率约为 9.9％。奥拉西坦作为一种神经保护药物，广泛应用于脑血管疾病导致的神经认知功能障碍。而奥拉西坦对于术后认知功能障碍相关炎性因子及相关标记物的影响在国内外也未见相关报道。如果本研究能够验证奥拉西坦可以改善老年腹腔镜胃肠手术术后谵妄发生率的科学假说，则可以预料，该方法将迅速应用于临床老年胃肠道手术的患者，可能是科研成果转化最快的项目之一。  

Objectives of Study：

Postoperative delirium is mainly found in elderly patients and has developed into one of the main complications after surgery in elderly patients, which seriously affects the quality of life after surgery. The incidence of POD was reported to be 25.8% in 7 days after non-cardiac surgery in elderly patients over 60 years of age, and about 9.9% in 3 months after surgery. As a neuroprotective drug, oxiracetam is widely used in neurocognitive dysfunction caused by cerebrovascular disease. The effects of oxiracetam on inflammatory factors and related markers associated with postoperative cognitive dysfunction have not been reported at home or abroad. If this study can verify the scientific hypothesis that oxiracetam can improve the incidence of delirium after laparoscopic gastrointestinal surgery in old age, it can be expected that this method will be applied quickly to patients with clinical intestinal surgery in old age, which may be one of the fastest-transforming projects in scientific research.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.ASAⅠ~Ⅲ级患者;2.年龄>75 岁;3.签署知情同意书;4.腹腔镜胃肠道手术，手术时间在 2-4 小时之间。

Inclusion criteria

1. ASAI~III patients; 2. Age>75 years old; 3. Sign an informed consent; 4. Laparoscopic gastrointestinal surgery.the operating time is between 2-4 hours.

排除标准：

1.合并严重心血管疾病、糖尿病、神经或精神疾病，肝肾功能不全;2.定期使用止痛药或手术前服用抗抑郁药;3.术前 MoCa 评分<23 分，认为术前存在认知障碍;4.无法遵守测试协议。

Exclusion criteria：

1.Combining severe cardiovascular disease, diabetes, neurological or mental illness, liver and kidney insanity; 2. Regular use of painkillers or antidepressants before surgery; 3. People with MoCa scores< 23 are considered as cognitive impairment before surgery; 4. People can not comply with the test agreement.

随机数字表由本院专业统计人员提供，利用 SAS 软件模拟产生。在本多中心研究中，三个研究中心的病例配置为 1:1:1，采用中心分层随机系统随机分配分中心的病例入组。各中心试验组和对照组病例数的比例应与总样本的比例相同，以保证各中心齐同可比。

Random number tables are provided by our professional statisticians and are simulated using SAS software.In this multicenter study, the cases of the three research centers were configured as 1:1:1,and the cases of the subcenter were randomly assigned using the central stratification random system.The proportion of cases in the experimental and control groups of each center should be the same as the proportion of the total sample to ensure that each center is homogeneous and comparable.
