研究目的:

To examine the effects of a nurse-led palliative care programme on improving psychological well-being (primary outcome) and secondary outcomes (spiritual well-being, meaning in life, symptom burden, psychological flexibility and distress related to death and dying) among patients with progressive neurological diseases (PNDs), compared with Befriending activity as active control.  

Objectives of Study:

To examine the effects of a nurse-led palliative care programme on improving psychological well-being (primary outcome) and secondary outcomes (spiritual well-being, meaning in life, symptom burden, psychological flexibility and distress related to death and dying) among patients with progressive neurological diseases (PNDs), compared with Befriending activity as active control.

药物成份或治疗方案详述:

Participants will receive five 90-minutes sessions of a nurse-led palliative care programme for PNDs, which aims to cultivating acceptance and promote values-based, meaningful living with progressive neurological diseases, or Befriending activity over three months. 

Description for medicine or protocol of treatment in detail:

Participants will receive five 90-minutes sessions of a nurse-led palliative care programme for PNDs, which aims to cultivating acceptance and promote values-based, meaningful living with progressive neurological diseases, or Befriending activity over three months. 

纳入标准:

1. Are diagnosed with neurodegenerative disease including multiples sclerosis, Parkinson’s disease, and spinocerebellar ataxia or neuromuscular disease including motor neuron disease, muscular dystrophy, and spinal muscular atrophy; 2. Score >= 8 on the Hospital Anxiety and Depression Scale-Depression subscale (sensitivity and specificity of approximately 0.80); 3. Aged 18 or above; 4. Fluent Cantonese speaker; 5. Able to sign an informed consent form.

Inclusion criteria

1. 1. Have severe cognitive impairment as indicated by the Abbreviated Mental Test lower than 6 (sensitivity and specificity of 0.94-0.96); 2. Are currently receiving psychotherapy/other behavioural interventions in the preceding six months; 3. Have prior experience in ACT-based intervention; 4. Have other contraindication or severe comorbidity that may limit their full participation (e.g. severe hearing or vision impairment).

Exclusion criteria:

First, the therapist takes a supportive–expressive approach in encouraging the open expression of thoughts and emotions and exploring issues including symptom control, relationships with healthcare providers, changes in self-concept, relationships with close others, spirituality and meaning in life, and future care planning. On the other hand, the session will troubleshoot the psychological inflexibility in the context of the client’s problems by incorporating the six core elements of ACT. The therapist will adopt the techniques of experiential exercises, mindfulness and metaphors.
