研究目的：

目前临床上有大量的研究优化无痛胃镜全身麻醉诱导的方式及用药。然而使用相同全身麻醉药物及剂量的前提下, 胃镜检查医师不同入镜时间点的选择，同样会对胃镜检查患者的不良反应，麻醉效果、术后并发症等情况产生较大影响。以往胃镜检查医生往往仅凭经验或通过睫毛反射消失选择进镜时机，随意性高、准确性差。脉搏末梢灌注指数在临床麻醉管理中，可以作为一个反映血管交感张力及麻醉深度的指标。相比于睫毛反射消失，PI具有简便、无创、客观、实时的优势，似乎更适合作为消化内镜诊疗的入镜时机。本研究通过对比不同的消化内镜进镜时的可行性、有效性及安全性，从而寻找无痛胃镜检查的最佳进镜时机，为临床操作提供参考。  

Objectives of Study：

A large number of studies have been conducted to optimize the induction of general anesthesia for painless gastroscopy. However, under the premise of using the same general anesthesia drugs and doses, the choice of different entry points for gastroscopy will also have a great impact on the adverse reactions, anesthetic effects, and postoperative complications of gastroscopy patients. In the past, gastroscopists often chose the time of entry only by experience or by the disappearance of the eyelash reflex, which was highly arbitrary and inaccurate. Pulse Perfusion Index (PI) can be used as an indicator of vascular sympathetic tone and anesthesia depth in clinical anesthesia management. Compared with the disappearance of the lash reflex, PI has the advantages of simplicity, noninvasiveness, objectivity, and real-time, and seems to be more suitable for the timing of admission to gastrointestinal endoscopy clinics. In this study, we compared the feasibility, effectiveness and safety of different gastrointestinal endoscopic entry times, so as to find the optimal entry time for painless gastroscopy and provide reference for clinical operation.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

择期行无痛胃肠镜检查的患者(ASA Ⅰ, Ⅱ)，心肺肝肾功能正常者。

Inclusion criteria

Patients undergoing elective painless gastroenteroscopy (ASA Ⅰ, Ⅱ) with normal cardiopulmonary, hepatic and renal functions.

排除标准：

BMI＞30kg/m2，发烧（耳温T≥37.5℃）、低体温者（耳温T≤36℃），原有严重高血压，心律失常或不稳定性心绞痛患者，急性上呼吸道感染或哮喘发作期患者，严重肝肾功能障碍患者，胃镜下摘息肉患者。

Exclusion criteria：

BMI > 30 kg/m2, fever (ear temperature T ≥ 37.5°C), hypothermia (ear temperature T ≤ 36°C), pre-existing severe hypertension, patients with cardiac arrhythmia or unstable angina pectoris, patients with acute upper respiratory tract infections or asthma exacerbation, patients with severe hepatic and renal dysfunction, and patients with gastroscopic polyp removal.
