研究目的：

膜性肾病(MN)是一种自身免疫性疾病，发病率近年来上升明显，自然病程预后差异较大，我们前期的研究发现肾小球局灶节段病变对膜性肾病预后有着重要的预测价值。近年研究研究发现壁层上皮细胞(PEC)在原发性局灶节段硬化的发生、进展中起到重要作用，PEC分子标记物也可以作为鉴别早期肾小球局灶节段硬化（FSGS）的标记物。原发性膜性肾病中，局灶节段病变是一种常见的病理改变，在膜性肾病中，目前国内外尚无关于局灶节段病变PEC标记物表达的研究，也无PEC标记物与膜性肾病预后的相关性研究。本研究拟通过分析PEC标记物claudin-1及CD44和IV型胶原α2链等在肾小球的表达，来研究PEC的活化、迁移及转分化，进而分析其在MN中局灶节段病变进展中的意义。  

Objectives of Study：

Membranous nephropathy (MN) is an autoimmune disease. The incidence rate has increased significantly in recent years, and the prognosis of the natural course of disease varies greatly. Our previous research found that focal segmental glomerular lesions have an important predictive value for the prognosis of membranous nephropathy. In recent years, research has found that parietal epithelial cells (PECs) play an important role in the occurrence and progression of primary focal segmental sclerosis, and PEC molecular markers can also serve as markers for distinguishing early glomerular focal segmental sclerosis (FSGS). In primary membranous nephropathy, focal segmental lesions are a common pathological change. Currently, there is no research on the expression of PEC markers in focal segmental lesions at home and abroad, nor is there a correlation between PEC markers and the prognosis of membranous nephropathy. This study aims to analyze the PEC marker claudin-1, CD44, and type IV collagen through analysis α To investigate the activation, migration, and transdifferentiation of PEC by studying the expression of 2-chain in the glomerulus, and to analyze its significance in the progression of focal segmental lesions in MN.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

原发性膜性肾病，规范随访时间不少于6个月。纳入伴有早期节段病变（EFSL+）组40例，伴有FSGS病变(FSGS+)40例，不伴有节段病变,即不伴早期节段病变及FSGS病变(FSL-)40例。选取病理科肾脏肿瘤切除标本周围相对未受侵犯的正常肾组织作为对照。

Inclusion criteria

Primary membranous nephropathy, with a follow-up period of no less than 6 months. 40 cases were included in the group with early segmental lesions (EFSL+), 40 cases with FSGS lesions (FSGS+), and 40 cases without early segmental lesions and FSGS lesions (FSL -). Select normal kidney tissue around the pathological kidney tumor resection specimen that is relatively unaffected as a control.

排除标准：

排除HBV、HCV相关性、自身免疫性疾病、肿瘤、药物等继发性肾小球膜性病变的患者，亦排除合并HBV、HCV病毒携带的非病毒相关性膜性肾病、糖尿病的患者。排除病理切片完整肾小球小于8个的患者。

Exclusion criteria：

Exclude patients with secondary glomerular membrane diseases such as HBV, HCV related, autoimmune diseases, tumors, drugs, and also exclude patients with non virus related membrane nephropathy and diabetes associated with HBV and HCV viruses. Exclude patients with less than 8 intact glomeruli in pathological sections.
