研究目的：

急性踝扭伤以后会出现疼痛、肿胀、关节活动受限、皮下瘀斑等症状，导致踝关节无法负重而影响活动。急性踝扭伤接受及时、正确的康复治疗后可以痊愈，否则可能导致踝关节周围韧带损伤加重，降低踝关节稳定性，最终发展为慢性踝关节不稳（chronic ankle instability，CAI）。据统计， 20%~40%的人在一次急性踝扭伤后会发展成CAI，这些人多伴有疼痛和反复踝扭伤，相较于急性踝扭伤，CAI对运动表现和日常生活造成更加严重的影响，而且会产生高昂的医疗费用，加重社会和个人的经济负担。体外冲击波作为一种快捷、方便的治疗方式，已被证实其可以改善局部血液循环，促进损伤部位的软组织修复，现已被应用于足底筋膜炎、骨延迟愈合、骨不连以及膝关节前交叉韧带损伤等的治疗中，并取得了良好的治疗效果，但对慢性踝关节不稳患者的潜在治疗效果还未被实验证明，本研究的目的就是探讨发散式体外冲击波治疗慢性踝关节不稳的疗效。  

Objectives of Study：

After an acute ankle sprain, symptoms such as pain, swelling, limited joint movement, and subcutaneous ecchymosis will appear, resulting in the inability of the ankle joint to bear weight and affecting activities. Acute ankle sprains can be cured after receiving timely and correct rehabilitation, otherwise it may lead to aggravated damage to the ligaments around the ankle joint, reduce the stability of the ankle joint, and eventually develop into chronic ankle instability (CAI). According to statistics, 20% to 40% of people will develop CAI after an acute ankle sprain. These people are often accompanied by pain and repeated ankle sprains. Compared with acute ankle sprains, CAI has more serious effects on sports performance and daily life. impact, and will generate high medical costs, increasing the economic burden on society and individuals. As a quick and convenient treatment, extracorporeal shock wave has been proven to improve local blood circulation and promote soft tissue repair at the damaged site. It has been applied to plantar fasciitis, delayed bone union, nonunion and knee joint In the treatment of anterior cruciate ligament injury, etc., and achieved good therapeutic effect, but the potential therapeutic effect on patients with chronic ankle instability has not been proved experimentally, the purpose of this study is to explore the divergent extracorporeal shock wave treatment of chronic ankle instability. stable curative effect.

纳入标准：

有至少 1 次需要医疗干预的严重脚踝扭伤病史；反复出现疼痛、肿胀、虚弱和不稳定的症状；反复发生崴脚；坎伯兰踝关节不稳定工具 (CAIT) 问卷得分≤25；

Inclusion criteria

History of at least 1 severe ankle sprain requiring medical intervention; recurrent symptoms of pain, swelling, weakness, and instability; recurrent sprains; Cumberland Ankle Instability Tool (CAIT) questionnaire score ≤25;

排除标准：

接受过患侧踝关节手术或有脚踝骨折、脱臼等病史；踝关节严重骨性关节炎、创伤性关节炎、剥脱性软骨炎等；踝关节周围有开放性损伤；不能接受治疗方案及不能配合治疗。

Exclusion criteria：
