研究目的：

1. 探究灌注指数比率及基于脉冲多普勒超声局部血流动力学参数早期评估超声引导下肌间沟臂丛神经阻滞效果的可行性,通过灵敏度和特异度的对比研究评估优效性； 2. 寻找影响灌注指数比率正确判断神经阻滞效果的危险因素。  

Objectives of Study：

1. To explore the feasibility of early evaluation of the effect of ultrasound-guided interscalene brachial plexus block based on perfusion index ratio and pulsed Doppler ultrasound regional hemodynamic parameters, and to evaluate the superiority by comparing sensitivity and specificity. 2. To find the risk factors affecting the perfusion index ratio to correctly judge the effect of nerve block.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 2023年1月至2023年12月于嘉兴学院附属第二医院择期行单侧关节镜下肩袖修补手术患者； 2. 年龄18-65岁； 3. ASA分级：I-II级。

Inclusion criteria

1. From January 2023 to December 2023, the patients underwent unilateral arthroscopic rotator cuff repair in the second affiliated hospital of Jiaxing University; 2. Aged 18-65 years; 3. ASA grade I-II.

排除标准：

1. 术前使用血管活性药物； 2. 术前使用镇痛药物； 3. 外周血管疾病； 4. 病态肥胖患者； 5. 存在感觉异常； 6. 周围神经阻滞禁忌症：包括患者拒绝、对局麻药过敏、周围神经病变、凝血功能障碍或存在精神症状。

Exclusion criteria：

1. Preoperative use of vasoactive drugs; 2. Preoperative use of analgesics; 3. Peripheral vascular diseases; 4. Morbically obese patients; 5. Paresthesia; 6. Contraindications to peripheral nerve block, including patient refusal, allergy to local anesthetics, peripheral neuropathy, agglutination disorder, or presence of psychiatric symptoms.

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

针刺法（25G）测定肌皮神经、桡神经、正中神经、尺神经支配区域的痛觉阻滞情况：即未阻滞（刺痛正常）、部分阻滞（刺痛减退）、完全阻滞（刺痛消失），①肌皮神经皮肤支配区：沿前臂外侧至腕部的皮肤区域；②桡神经绝对皮肤支配区：手背侧虎口区；③正中神经绝对皮肤支配区：食、中指末节；④尺神经绝对皮肤支配区：小指。 以上述各神经的评估最低级别为阻滞效果。阻滞效果2级为痛觉完全阻滞,判断为阻滞成功。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Acupuncture method (25G) was used to determine the nociceptive block of musculocutaneous nerve, radial nerve, median nerve and ulnar nerve innervation area: no block (tingling normal), partial block (tingling decreased), complete block (tingling disappeared).
