研究目的：

通过评估不稳定性老年股骨粗隆间骨折术前使用LMWH预防VTE的获益及风险，明确该类患者的最佳抗凝时机。  

Objectives of Study：

To determine the best time for anticoagulation in elderly patients with unstable intertrochanteric fracture in perioperative period.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄大于或等于65岁； 2. 不稳定性股骨粗隆间骨折行髓内钉内固定手术治疗者(不稳定性股骨粗隆间骨折定义为AO/OTA 分型中的A2.2-3.3型)。

Inclusion criteria

1. Age 65 or older; 2. Unstable intertrochanteric fracture is treated by intramedullary nail internal fixation (unstable intertrochanteric fracture is defined as A2.2-3.3 in AO/OTA classification).

排除标准：

1. 术前检查发现存在下肢深静脉血栓形成； 2. 术前血红蛋白小于80g/L； 3. 病理性骨折； 4. 多发性骨折，另一部位骨折需手术治疗； 5. 有抗凝治疗禁忌症； 6. 伤后至入院时间大于3天。

Exclusion criteria：

1. Preoperative examination revealed deep venous thrombosis of the lower extremities; 2. The preoperative hemoglobin is less than 80g/L; 3. Pathological fracture; 4. Multiple fractures with another part of the fracture needs surgical treatment; 5. There are contraindications for anticoagulant therapy; 6. The time from injury to admission is more than 3 days.
