研究目的: |
以早期脑损伤、癫痫性脑病、癫痫持续状态、孤独症谱系障碍、颅内感染的病人为对象,以临床、神经影像、神经电生理、遗传学和食品组学方法,设立对照,多中心的设计进行生酮饮食早期干预效果分析,旨在探索预防效果及其生物标志物,早期治疗前预测效果的生物标志物,为实现早期预防、精准治疗脑损伤、癫痫等儿童神经系统疾病提供依据。 |

Objectives of Study: |
Patients with early brain injury, epileptic encephalopathy, status epilepticus, autism spectrum disorder, and intracranial infection were selected as subjects. Clinical, neuroimaging, neuroelectrophysiological, genetic, and food omics methods were used with a control group, and a multicenter design was designed to explore the preventive effect and biomarkers of ketogenic diet. Biomarkers that predict the effect before early treatment can provide a basis for early prevention and precise treatment of brain injury, epilepsy and other neurological diseases in children. |

|
纳入标准: |
(1)生酮饮食预防脑损伤早产儿等高危儿发生癫痫,入组干预和对照组各76例。 (2)癫痫持续状态早期介入,缩短住院时间减少后遗症,入组干预和对照各48例。 (3)早期治疗癫痫性脑病减少残疾,预测生物标志物,暴露和非暴露各94例。 (4)预防和早期改善孤独症谱系障碍核心症状,干预和非干预各68例。 (5)颅内感染辅助治疗,减少并发症和后遗症,免疫学机制,干预和对照各130例。 探索规范化治疗流程或制定合适的评估方法。诊断标准:诸福棠实用儿科学,第9版,以及最新专科指南。各子题分别的纳、排标准: 1)早产儿等脑损伤高危儿 纳入标准:入组标准:新生儿科住院早产儿等高危儿(孕龄不足37周),性别不限,年龄出生2个月内有先天、后天脑损伤疾病,包括基因相关、结构、代谢、感染、免疫等,或病因未明。 2)生酮饮食早期辅助治疗癫痫持续状态 入组标准:按国际抗癫痫联盟2015年诊断标准,惊厥性癫痫持续状态,入组年龄0-18岁,性别不限。 3)生酮饮食早期辅助治疗癫痫性脑病 入组标准:根据2017-2022版国际抗癫痫联盟癫痫诊断分类标准,新诊断为婴儿癫痫性痉挛综合征(IESS)未经治疗,性别不限,2岁以内起病。 4)生酮饮食预防和早期改善孤独症谱系障碍核心症状 入组标准:按中国低龄儿童孤独症谱系障碍早期诊断专家共识2022版诊断孤独症谱系障碍,18岁以下,性别不限。 5)生酮饮食辅助颅内感染辅助治疗 入组标准:按儿童社区获得性细菌性脑膜炎诊断与治疗专家共识2019版诊断细菌性脑膜炎,有病原学依据或脑脊液白细胞500*10^6/L以上,年龄18岁以下,性别不限。 |

Inclusion criteria |
(1) Ketogenic diet was used to prevent epilepsy in preterm or other high risk infants with brain injury, 76 cases in the intervention group and 76 cases in the control group. (2) Early intervention of status epilepticus shortened the length of hospital stay and reduced the sequelae. Ninety-four patients were enrolled in the intervention group and 48 patients in the control group. (3) Early treatment of epileptic encephalopathy to reduce disability, predictive biomarkers, exposed and unexposed 94 cases each. (4) Prevention and early improvement of core symptoms of autism spectrum disorder, intervention and non-intervention 68 cases respectively. (5) Adjuvant treatment of intracranial infection, reduction of complications and sequelae, immunological mechanism, intervention and control of 130 cases each. To explore the standardized treatment process or develop appropriate evaluation methods. Diagnostic criteria: Zhu Futang Practical Pediatrics, 9th edition and updated guideline.The inclusion and exclusion criteria of each subquestion are as follows: 1) Premature infants and other high-risk infants with brain injury inclusion criteria: The inclusion criteria were hospitalized in the department of Neonatology (gestational age < 37 weeks), both male and female, with congenital or acquired brain injury diseases within the age of 2 months, including gene related, structural, metabolic, infectious, immune, etc., or the etiology was unknown. 2) Status epilepticus Inclusion criteria: according to the International League Against Epilepsy 2015 diagnostic criteria, convulsive status epilepticus, age 0-18 years old, regardless of gender. 3)Ketogenic diet as an early adjuvant therapy for epileptic encephalopathy Inclusion criteria: According to the 2017-2022 edition of the International League Against Epilepsy diagnostic classification criteria, newly diagnosed infantile epileptic spasm syndrome (IESS) was untreated, with onset within 2 years of age, of both boys and girls. 4) Ketogenic diet for prevention and early improvement of core symptoms of autism spectrum disorder (ASD) Inclusion criteria: ASD was diagnosed according to the Chinese Expert consensus on the Early diagnosis of ASD in Young children 2022 edition, children under 18 years of age, regardless of gender. 5) Ketogenic diet for intracranial infection: Inclusion criteria: patients were diagnosed with bacterial meningitis according to the expert consensus on the diagnosis and treatment of community-acquired bacterial meningitis in children (2019 edition), with etiological evidence or cerebrospinal fluid WBC over 500*10^6/L, under 18 years old, regardless of gender. |

排除标准: |
生酮饮食的禁忌症 ①绝对禁忌症:肉碱缺乏症(主要为原发性;肉碱棕榈酰基转移酶(CPT)I和II缺乏症;肉碱移位酶缺乏症;β-氧化障碍;中链酰基脱氢酶缺乏症(MCAD);长链酰基脱氢酶缺乏症(LCAD);短链酰基脱氢酶缺乏症(SCAD);长链3 -羟酰辅酶A缺乏症;中链3 -羟酰辅酶A缺乏症;丙酮酸羧化酶缺乏症;卟啉病 ②相对禁忌症:不能维持适量营养的患者;父母或监护者不配合;异丙酚同时使用(异丙酚输注综合征可能风险更高,建议相隔24小时)。 各子题的禁忌证: 1)生酮饮食防治早产儿等脑损伤高危儿的癫痫 排除标准:早产儿等高危儿出生后2个月内已诊断为新生儿癫痫或癫痫综合征的;严重心、肺、肝、肾等器官或全身性疾病,生命不稳定或严重功能障碍的;先天性心脏病,影响心功能的,有生酮饮食疗法禁忌证的。 2)生酮饮食早期辅助治疗癫痫持续状态 排除标准:严重心、肺、肝、肾等器官或全身性疾病,生命不稳定或严重功能障碍的;有生酮饮食疗法禁忌证的。 3)生酮饮食早期辅助治疗癫痫性脑病 排除标准:严重心、肺、肝、肾等器官或全身性疾病,生命不稳定或严重功能障碍的;有生酮饮食疗法禁忌证的。 4)生酮饮食预防和早期改善孤独症谱系障碍核心症状 排除标准:严重心、肺、肝、肾等器官或全身性疾病,生命不稳定或严重功能障碍的;有生酮饮食疗法禁忌证的;当前无法治疗的罕见疾病导致孤独症谱系障碍的。 5)生酮饮食辅助颅内感染辅助治疗 排除标准:严重心、肺、肝、肾等器官或全身性疾病,严重并发症,导致生命不稳定或严重功能障碍的;有生酮饮食疗法禁忌证的。 |

Exclusion criteria: |
Contraindications to the ketogenic diet: ① Absolute contraindications: carnitine deficiency (mainly primary; Carnitine palmitoyltransferase (CPT) I and II deficiency; Carnitine translocase deficiency; β-oxidation disorders; Medium-chain acyl dehydrogenase deficiency (MCAD); Long-chain acyl dehydrogenase deficiency (LCAD); Short-chain acyl dehydrogenase deficiency (SCAD); Long-chain 3-hydroxyacyl-coa deficiency; Medium-chain 3-hydroxyacyl-coa deficiency; Pyruvate carboxylase deficiency; Porphyria. ② Relative contraindications: patients who cannot maintain proper nutrition; Uncooperative parents or guardians; Propofol is administered simultaneously (possibly at higher risk for the propofol infusion syndrome, 24 hours apart is recommended). Exclusion criteria for each project as follows: 1) Exclusion criteria for epilepsy in preterm infants and other high-risk infants with brain injury: those diagnosed with neonatal epilepsy or epileptic syndrome within 2 months after birth; Serious heart, lung, liver, kidney and other organs or systemic diseases, life instability or serious dysfunction; Congenital heart disease, affecting cardiac function, and contraindications to ketogenic diet therapy. 2) Ketogenic diet for status epilepticus exclusion criteria: severe heart, lung, liver, kidney and other organ or systemic diseases, unstable life or severe dysfunction; Contraindications to ketogenic diet therapy. 3) Exclusion criteria for early ketogenic diet treatment of epileptic encephalopathy: severe heart, lung, liver, kidney and other organ or systemic diseases, unstable life or severe dysfunction; Contraindications to ketogenic diet therapy. 4) The exclusion criteria for the prevention and early improvement of the core symptoms of ASD were as follows: severe heart, lung, liver, kidney and other organ or systemic diseases, unstable life or severe dysfunction; Patients with contraindications to ketogenic diet therapy; Currently incurable rare diseases that cause autism spectrum disorders. 5) Exclusion criteria in intracranial infection project with adjunct ketogenic diet therapy: severe heart, lung, liver, kidney and other organ or systemic diseases, serious complications, leading to life instability or serious dysfunction; Contraindications to ketogenic diet therapy. |
