研究目的：

癌因性疲乏（简称CRF）是肿瘤患者常见症状之一，放化疗期间症状显著加重。研究发现，超过70%的肺癌化疗患者伴有疲乏，严重影响其生活质量。目前，CRF的发病机制尚不明确，仍缺乏有效的缓解手段，而中医外治法在CRF治疗中显示出较明显的优势。文献资料和项目组前期研究发现，中药热熨敷技术能有效缓解CRF症状，但相关研究均处于起步阶段，存在研究较少、针对性不强、研究设计欠规范，且鲜有机制探讨等问题。技术应用也存在器具不便利、操作细节需优化等问题。本研究选择第2及以上周期化疗伴轻/中度CRF肺癌患者为研究对象，采用前瞻性、多中心、大样本、随机对照临床实验研究方法，干预组在对照组护理方法的基础上，应用结合肺癌化疗患者特点的中药热熨敷技术方案，通过分析疲乏程度、生存质量、KPS评分的改善情况、不良事件发生率，评价中药热熨敷技术对CRF的作用效果及安全性；分析血液相关免疫指标，探讨中药热熨敷干预CRF的作用机制，以期形成高质量的研究证据及可推广的技术方案。  

Objectives of Study：

Cancer-induced fatigue (CRF) is one of the common symptoms in cancer patients, which is significantly aggravated during radiotherapy and chemotherapy. Some studies have found that more than 70 percent of lung cancer patients treated with chemotherapy are associated with fatigue, which severely affects their quality of life. At present, the pathogenesis of CRF is still unclear, and there is still a lack of effective palliative means, while the external treatment of traditional Chinese medicine has shown obvious advantages in the treatment of CRF. Literature and preliminary research of the project team found that TCM Hot Ironing Compress can effectively relieve the symptoms of CRF. But relevant studies are still in the initial stage, with few studies, weak pertinency, non-standard study design, and few mechanism discussions. There are also some problems in technical application, such as inconvenient appliances and optimized operation details. In this study, patients with lung cancer accompanied by mild/moderate CRF during the second or above cycle of chemotherapy were selected as the research objects, and a prospective, multi-center, large-sample, randomized controlled clinical trial was adopted. The intervention group, on the basis of the nursing method of the control group, was applied TCM Hot Ironing Compress combined with the characteristics of lung cancer chemotherapy patients. The effect and safety of TCM Hot Ironing Compress technique on CRF were evaluated by analyzing fatigue degree, quality of life, improvement of KPS score and incidence of adverse events. The blood related immune indexes were analyzed to explore the mechanism of Chinese medicine hot ironing in the intervention of CRF, in order to form high-quality research evidence and technical schemes that can be promoted.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①病理学检查确诊为肺恶性肿瘤患者； ②符合ICD-10癌症相关疲乏的诊断标准，Piper 疲乏量表中文修订版评分≤6分，即轻、中度疲乏； ③病情稳定； ④KPS≥60 分； ⑤18 岁<年龄<80 岁； ⑥已结束第一周期化疗，正在接受第二及以上周期化疗的患者； ⑦入组前一周内未接受过中药热熨敷技术治疗； ⑧患者可积极配合干预治疗，无意识障碍，可完成量表填写，并签署知情同意书。

Inclusion criteria

① Patients diagnosed with lung malignant tumor by pathological examination; ② Meeting the ICD-10 diagnostic criteria for cancer-related fatigue, the Piper Fatigue Scale (Chinese revised version) score ≤6, namely, mild to moderate fatigue; ③ Stable condition; ④ KPS 60 or more points; ⑤18 years old＜AGE <80 years old; ⑥ Patients who have finished the first cycle of chemotherapy and are receiving the second or above cycle of chemotherapy; ⑦ No TCM Hot Ironing Compress technique was used in one week before enrollment; ⑧Patients can actively cooperate with intervention treatment, unconscious disorder, can complete the scale, and sign informed consent.

排除标准：

①合并严重的心、肝、肾功能异常； ②出、凝血功能异常； ③局部皮肤炎症、疤痕、破损； ④正在使用生血丸/参芪扶正注射液/贞芪扶正颗粒等扶正益气类中药或保健品，影响疗效判定者； ⑤正在接受同步放疗的患者； ⑥其他中药热熨敷的禁忌证。

Exclusion criteria：

① Complicated with serious abnormality of heart, liver and kidney function; ② Abnormal bleeding and coagulation function; ③ Local skin inflammation, scar, damage; ④ Those who are using shengxue pill, Shenqi Fuzheng Injection, Zhenqi Fuzheng granule and other fuzheng supplementing traditional Chinese medicine or health care products, affecting the efficacy evaluation; ⑤ Patients undergoing synchronous radiotherapy; ⑥ Other contraindications of TCM Hot Ironing Compress.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.数据采集： 数据的采集工作由经过系统培训及考核的临床实施组完成采集及CRF上所有数据的填写。 2.数据管理： 患者观察周期结束，CRF表所有项目均填写完毕后，应先交由数据管理组成员双人核对录入电子版病例记录表及ResMan临床试验公共管理平台。整个项目的数据管理工作由数据管理组统一负责。所有纸质版数据均统一安全存放，电子版数据设置打开密码存放至数据管理组电脑，打开密码不可外漏。

1. Data collection: Data collection was completed by the clinical implementation group that had been systematically trained and assessed and all data were filled in on the CRF. 2. Data Management: After the patient observation period is over and all items in the CRF form are filled in, it should be checked by two members of the data management team and entered into the electronic case record form and ResMan Clinical Trial Public Management platform. The data management team is responsible for the data management of the whole project. All paper data are stored in a unified and safe manner, and the password for opening electronic data is stored in the computer of the data management group, and the password cannot be leaked.
