探索CT引导下经皮腰交感神经节射频/臭氧毁损术治疗难治性高血压的临床有效性和安全性。  

Objectives of Study：

To determine whether CT-guided ozone-mediated lumbar sympathetic denervation (OL-SDN) displays the clinical efficacy and safety profile in patients with resistant hypertension (RH).

药物成份或治疗方案详述：

患者按预定位置放置在CT台上，监测心率和血压。CT扫描后，确定建议的穿刺位置为椎前神经节，L2上缘水平与腰大肌的前内侧区域和腹主动脉的背外侧相邻，为左肾动脉的起点。测量穿刺深度和角度。2%利多卡因局部麻醉后，21号针头推进至目标点，再次行CT扫描确认。沿针头注入浓度为40 g/ml的13 ml臭氧和氧气的混合物。手术结束后，取出穿刺针。注射后5min第三次CT扫描可见局部臭氧弥散。每5分钟监测1次患者的血压，持续30分钟，然后患者返回医院病房。 

Description for medicine or protocol of treatment in detail：

CT-guided ozone-mediated lumbar sympathetic denervation (OL-SDN) The patient is placed on the CT table in a pre-position, and the heart rate and BP are monitored. After a single CT scan (General ElectricsCompany,Hi Speed ZX/i), the proposed needle tip location was determined to be the prevertebral ganglia, adjacent to the anteromedial area of the psoas muscle and the dorsolateral side of the abdominal aorta at the level of the upper edge of L2, where the left kidney artery originates. The depth and angle of the puncture were measured. After local anesthesia with 2% lidocaine, 21 G needles (Japan's Hakko Company) advanced to the target point and were confirmed by another CT scan. After testing the return pump and confirming that it was blood-free and liquid-free, a mixture of 13 ml of ozone and oxygen gas was injected along the needle with a concentration of 40 g/ml. After the procedure, the puncture needle is removed. The diffusion of ozone was observed in the last CT scan five minutes after the injection. The patient's blood pressure was monitored once every five minutes for 30minutes, and he returned to the hospital ward. 

纳入标准：

1.在进行任何评估和治疗之前，必须获得患者书面知情同意。 2.患者男女不限，年龄18 ~ 80岁。 3.根据美国心脏协会的科学声明确诊难治性高血压(RH)，即在服用包括一种利尿剂在内的至少三种最大耐受剂量的抗高血压药时，24小时动态血压监测的平均收缩压≥130 mmHg和/或白天时间的平均收缩压≥135 mmHg; 或使用四种或更多的抗高血压药控制血压。

Inclusion criteria

1.Written informed consent must be obtained before any assessment is performed.
2.Male and female patients, age 18 to 80 years inclusive
3.Confirmed diagnosis of resistant hypertension (RH) in accordance with the American Heart Association scientific statement, i.e.to have a mean systolic blood pressure of ≥130 mmHg on 24-hour ambulatory blood pressure monitoring and/or ≥135 mm Hg during the day time hours, while taking maximally tolerated doses of at least three antihypertensive agents including a diuretic; or to have a controlled blood pressure with four or more antihypertensive agents.

排除标准：

1.确诊继发性高血压患者(肾动脉狭窄;肾上腺嗜铬细胞瘤;库欣病;原发性醛固酮增多症;阻塞性睡眠呼吸暂停综合征(OSAS))。 2.脊柱畸形、脊柱感染性疾病患者。 3.有肾动脉血管成形术或去神经术史的患者。 4.有腰椎内椎间盘射频消融术和交感神经阻滞史的患者。 5.心肌梗死/脑血管意外(CVA/短暂性脑缺血发作(TIA)病史

Exclusion criteria：

1.Patients with confirmed secondary hypertension (all-caused renal artery stenosis; adrenal pheochromocytoma; cushing disease; primary hyperaldosteronism; occluded sleep apnea syndrome (OSAS) ).
2.Patients with spine malformation, spine infective disease.
3.Patients with history of renal artery angioplasty or denervation.
4.Patients with history of lumber internal vertebra disc frequency ablation and sympathetic blockage.
5.Myocardial infarction / Cerebral Vascular Accident (CVA) / TIA within the past three months prior to enrollment.
