研究目的：

（1）构建基于Triangle管理理论的老年痴呆分层分级管理模型 （2）验证该方案对老年痴呆患者及其居家照顾者的应用效果。  

Objectives of Study：

( 1 ) Construct a hierarchical management model of Alzheimer 's disease based on Triangle management theory. ( 2 ) To verify the application effect of the program on senile dementia patients and their home caregivers.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者纳入标准：①年龄在60岁及以上；②符合WHO国际疾病分类（ICD-10）诊断标准，诊断为AD患者。 照顾者纳入标准：①年龄≥18岁；②小学及以上文化，具有一定的阅读、理解及沟通能力，能熟练使用智能手机；③为AD患者的主要照顾者：连续居家照护时间>3个月且每周照顾天数>5天的亲属照顾者（包括配偶、子女或其他近亲）；④所有照顾者对本次研究知情，并签署“知情同意书”；⑤无重大躯体疾病和精神疾病；

Inclusion criteria

Inclusion criteria : 1 patients aged 60 years and above ; 2 Patients were diagnosed as AD according to the diagnostic criteria of WHO International Classification of Diseases ( ICD-10 ). Caregiver inclusion criteria : 1 age ≥ 18 years old ; 2 Primary school and above culture, with a certain reading, understanding and communication skills, can use smart phones ; ( 3 ) Main caregivers of AD patients : family caregivers ( including spouses, children or other close relatives ) with continuous home care time > 3 months and weekly care days > 5 days ; 4 All caregivers were informed of the study and signed the ' informed consent form ' ; 5 no major physical illness and mental illness ;

排除标准：

患者排除标准：①病重、病危或病情不稳定的终末期患者。 患者剔除标准：①干预期间出现了严重疾病如严重脏器损害疾病、心脑血管疾病等需要紧急住院。 照顾者排除标准：①曾经参加过或正在参加类似照护培训研究；②有照护报酬；③公益服务自愿者；④在可预见的将来（研究期限内）不再提供照顾者。 照顾者剔除标准：①研究中途要求终止或搬迁等原因无法联系照顾者；

Exclusion criteria：

Exclusion criteria : 1 End-stage patients with severe, critical or unstable disease. Exclusion criteria : 1 Patients with severe diseases such as severe organ damage, cardiovascular and cerebrovascular diseases need emergency hospitalization during the intervention period. Exclusion criteria for caregivers : 1 Have participated in or are participating in similar care training studies ; 2 care remuneration ; 3 Volunteers of public service ; 4 Caregivers will not be provided in the foreseeable future ( within the study period ). Caregiver exclusion criteria : 1 Caregivers could not be contacted for reasons such as termination or relocation during the study ;

本研究由研究人员按照AD疾病分期分为轻、中及重度3层，根据实际纳入疾病分期情况计算每层抽取的数量，再由非本课题组成员运用Research Randomizer网站产生一套随机数字序列，然后将这些随机数字分别装入按顺序编码、密封、不透光的信封中，将符合标准的研究对象进行纳入后再进行简单随机抽样。

Randomization Procedure (please state who generates the random number sequence and by what method)：

In this study, the researchers divided the AD disease into three levels : mild, moderate and severe. According to the actual inclusion of the disease stage, the number of extractions from each layer was calculated, and then a set of random number sequences were generated by non-members of the research group using the Research Randomizer website, and then these random numbers were loaded into sequential coding, sealed, and opaque envelopes. The eligible subjects were included and then subjected to simple random sampling.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

通过东华电子病历系统采集，采用Epi Data3.1及时录用数据，并限定条件，所有患者资料的收集都经2人核对后录入Excel数据库，进行计算机检索数据，防止录入错误。对有疑问的数据及时查看原始资料并更正，查漏补缺，空白数据追溯原病例系统，录入的数据由专人整理、保存，避免出现信息偏移，数据分析由1名具有统计学专业的研究生进行分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Through the collection of Donghua electronic medical record system, Epi Data3.1 was used to record data in time, and the conditions were limited. The collection of all patient data was checked by 2 people and entered into Excel database to retrieve data by computer to prevent input errors. The original data were checked and corrected in time for the doubtful data, and the blank data were traced back to the original case system. The input data were sorted out and saved by a special person to avoid information deviation. The data analysis was analyzed by a graduate student majoring in statistics.
