通过对胸横肌平面-胸部神经的联合阻滞在全身麻醉下胸腔镜肺癌根治术患者中应用，旨在明确胸横肌平面-胸部神经多种神经阻滞的优势，为癌症患者围术期提供一种操作简单、并发症少的麻醉管理思路，并最终提高患者满意度、节约医疗资源，减轻患者医疗负担。  

Objectives of Study：

Of thoracic transverse plane - chest nerve block thoracoscope under general anesthesia in patients with lung cancer radical application, aims to clear the chest transverse plane - chest the advantages of a variety of nerve block for cancer patients perioperative operation to provide a simple, less complications of anesthesia management of ideas, and ultimately improve patient satisfaction, save medical resources, reduce the patients' medical burden

药物成份或治疗方案详述：

胸横肌平面是与腹横肌平面相延续，位于胸横 肌和肋间内肌的肌间隙，T2-T6肋间神经前支从此处穿出，支配内侧区域皮肤感觉，局麻药可在该肌肉间隙上下获得良好的扩散，从而阻滞多个节段 肋间神经前支，达到镇痛效果。Ueshima 等首次使用并报道胸横肌平面-胸部神经阻滞用于危重乳腺癌手术患者能有效地阻滞 T2 -T6肋间神经前支。 相关文献表明，胸横肌平面阻滞范围为前胸正中区域 T2-T6 肋间神经前皮支，在胸横肌平面注射一定量的局部麻醉药物后，可看到药液在胸横肌与肋间内肌 及肋软骨的下方均匀扩散。同时有尸体解剖的 研究发现，在超声引导下将稀释的亚甲蓝溶液 20 ml 注入尸体的胸横肌平面，溶液可均匀分布到 T2-T6 平面，亚甲蓝染色范围与胸横肌的解剖位置十分贴切，这是胸横肌平面阻滞的解剖基础。 

Description for medicine or protocol of treatment in detail：

The transverse thoracic muscle plane is located in the muscle gap between the transverse thoracic muscle and the internal intercostal muscle, which extends to the transverse abdominis muscle plane. The anterior branch of the T2-T6 intercostal nerve penetrates from this point and innervates the skin sensation in the medial area. Local anesthetics can obtain good diffusion in the muscle gap, so as to block the anterior branch of multiple segmental intercostal nerves and achieve analgesic effect. Ueshima et al. first used and reported that transversus thoracic plane-thoracic nerve block can effectively block the anterior branch of T2-T6 intercostal nerve in critical breast cancer patients. Relevant literature showed that the block range of the transverse thoracic muscle plane was the anterior cutaneous branch of the T2-T6 intercostal nerve in the median area of the anterior chest. After a certain amount of local anesthetic was injected into the transverse thoracic muscle plane, the drug solution could be seen to evenly spread under the transverse thoracic muscle, intercostal muscles and costal cartilage. At the same time, some autopsy studies found that under the guidance of ultrasound, 20 ml diluted methylene blue solution was injected into the plane of the transverse thoracic muscle of the corpse, and the solution could be evenly distributed to the T2-T6 plane. The range of methylene blue staining was very close to the anatomical position of the transverse thoracic muscle, which was the anatomical basis of the transverse thoracic muscle plane block. 

纳入标准：

①同意接受胸腔镜下肺癌根治术，自愿参加本研究并签署知情同意书；②美国麻醉医师协（ASA）分级I~II级；③年龄18-70岁。

Inclusion criteria

① Agreed to undergo thoracoscopic radical resection of lung cancer, volunteered to participate in this study and signed informed consent; ② American Society of Anesthesiologists (ASA) grade I~II; ③ Ages 18-70.

排除标准：

①气管支气管先天性解剖结构变异；②重度肥胖( BMI > 35 kg / m2 )；③ASA≥Ⅲ级；④中重度阻塞性或限制性通气功能障碍；⑤心功能不全；⑥妊娠；⑦对局麻药过敏；⑧血小板和凝血功能异常；⑨畸形或感染；⑩患精神病；?短期或长期使用阿片类药物；?神经功能障碍；?无法了解NRS评分及自控静脉镇痛( PCIA )的使用情况。

Exclusion criteria：

① Congenital anatomical variation of trachea and bronchus; ② Severe obesity (BMI > 35 kg/m2); (3) ASA Ⅲ or higher level; ④ Moderate to severe obstructive or restrictive ventilatory dysfunction; ⑤ Cardiac insufficiency; 6 pregnancy; ⑦ Allergic to local anesthetics; ⑧ abnormal platelet and coagulation function; ⑨ deformity or infection; ⑩ Mental illness; Short-term or long-term use of opioids; Qiu nerve dysfunction; The usage of NRS score and controlled intravenous analgesia (PCIA) was unknown.
