研究目的：

（1）了解老年脑卒中患者目前的心理健康水平，探讨焦虑抑郁情绪对老年脑卒中患者生活质量的影响； （2）通过对老年脑卒中患者实施健康教育干预，分析该干预措施的效果及推官性； （3）解决护理评估与护理计划的脱节现状，改善脑卒中患者的长期预后。为日后老年慢性病的管理提供理论参考。  

Objectives of Study：

(1) Understand the current mental health level of elderly stroke patients, and explore the impact of anxiety and depression on the quality of life of elderly stroke patients; (2) by implementing health education interventions for elderly stroke patients, the effect and promotion of the intervention were analyzed; (3) Solve the disconnect between nursing assessment and nursing planning, and improve the long-term prognosis of stroke patients. It provides a theoretical reference for the management of chronic diseases in the elderly in the future.

药物成份或治疗方案详述：

依照干预映射框架构建出院后自我照护方案，分别于出院当日、出院后的第一个月、第三个月急进行随访干预。假设基于干预映射的出院自我照护方案的可改善脑卒中患者出院后3个月内焦虑抑郁水平以及生活质量。 

Description for medicine or protocol of treatment in detail：

The post-discharge self-care programme was constructed according to the intervention mapping framework, and follow-up interventions were conducted on the day of discharge, the first month and the third month after discharge. It was hypothesised that the mapping-based discharge self-care programme would improve the anxiety and depression levels and quality of life of stroke patients 3 months after discharge. 

纳入标准：

纳入标准
（1）年龄≥60岁；
（2）符合《中国急性缺血性脑卒中诊治指南（2018）》相关诊断标准，经头颅CT或MRI证实为脑卒中；
（3）意识清醒可自行表达，对答流利者；
（4）焦虑情绪评分（HAMA评分）≥7分；
（5）抑郁情绪评分（HAMD评分）≥17分；
（6）同意接受出院后3个月随访干预者；
（7）在贵阳市居住者；

Inclusion criteria

Inclusion criteria
(1) Age ≥ 60 years.
(2) Meeting the relevant diagnostic criteria of the Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke (2018), with stroke confirmed by cranial CT or MRI.
(3) Conscious and self-expressive with fluent replies.
(4) Anxiety score (HAMA score) ≥7.
(5) depressed mood score (HAMD score) ≥ 17
(6) Consent to follow-up intervention 3 months after discharge.
(7) Those who lived in Guiyang city.

排除标准：

排除标准
（1）存在严重神经精神疾病者，无法完成干预的患者；
（2）存在合并存在心肝肾等重要脏器功能障碍者；
（3）卒中前已确诊焦虑症和/或抑郁症患者。

Exclusion criteria：

Exclusion criteria
(1) Patients with severe neuropsychiatric disorders who are unable to complete the intervention.
(2) Patients with coexisting critical organ dysfunction such as heart, liver and kidney
(3) Patients with a pre-stroke diagnosis of anxiety and/or depression.
3 Exclusion criteria
(1) Those who are unable to cooperate during the intervention to complete the intervention.
(2) Patients who were unable to be contacted in time for follow-up or who were unable to come to the hospital for review; post-discharge health education interventions.
