探究围术期静脉使用艾司氯胺酮对腹腔镜下行妇科手术患者焦虑、抑郁水平的影响  

Objectives of Study：

Exploring the effect of perioperative intravenous use of Esketamine on anxiety and depression levels in patients undergoing laparoscopic gynecological surgery

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）预计腹腔镜下行妇科手术患者； 2）ASA分级Ⅰ~Ⅱ级； 3）年龄18岁~75岁的患者； 4）无沟通障碍，意识清晰； 5）自愿签订知情同意书

Inclusion criteria

1) Expected patients undergoing gynecological surgery under laparoscopy; 2) ASA grading I to II; 3) Patients aged 18-75 years old; 4) No communication barriers, clear consciousness; 5) Voluntary signing of informed consent form

排除标准：

1) 严重高血压、严重心血管疾病者； 2) 肺动脉高压、颅压或眼内压过高者 3) 慢性疼痛史； 4) 精神病史、中枢神经系统损伤； 5) 精神类药物使用史或酒精滥用史； 6) 严重肝肾功能不全者； 7) 沟通障碍；

Exclusion criteria：

1) Individuals with severe hypertension or cardiovascular disease; 2) Patients with pulmonary hypertension, intracranial pressure, or high intraocular pressure 3) History of chronic pain; 4) History of mental illness, central nervous system injury; 5) History of psychotropic drug use or alcohol abuse; 6) Severe liver and kidney dysfunction; 7) Communication barriers;

麻醉诱导：使用艾司氯胺酮0.5mg/kg静注。术中维持：使用艾司氯胺酮0.25mg/(kg*h)静脉泵注。术后镇痛：使用PCIA--艾司氯胺酮0.15mg/kg+舒芬太尼3ug/kg+托烷司琼10mg，加0.9%NaCl溶液配至100ml，单次输注0.5ml，持续速率2ml/h，锁定时间15min。

Intervention：

Anesthesia induction: esketamine 0.5mg/kg intravenously. Intraoperative maintenance: esketamine 0.25mg/(kg*h) intravenous pumping. Postoperative analgesia: PCIA-esketamine 0.15mg/kg + sufentanil 3ug/kg + toxieston 10mg, add 0.9% NaCl solution to 100ml, a single infusion of 0.5ml, continuous rate 2ml/h, locking time 15min.
