研究目的:

COVID-19是一种由严重急性呼吸系统综合征引起的新型疾病,越来越多的研究发现,微血栓形成是新冠肺炎的显著临床特征。肺循环微血栓形成与肺部渗出同时存在,将导致肺循环阻力增加,肺动脉压力升高。我国约有0.8亿人居住高海拔地区,5%-10%居民因居住于高海拔地区因失去高原适应能力而患有慢性高原疾病,高原肺动脉高压就是典型代表,表现为中度低氧血症、存在或不存在右心肥厚和右心衰竭,并伴有红细胞增多症。所以,在高原居住的人群出现新冠病毒感染后,对于合并高原性肺动脉高压、红细胞增多症或者COPD等肺部疾病的病例将进一步导致肺循环阻力增加,发生严重肺动脉高压甚至导致肺源性心脏病。我们希望旨在重型、危重型以及合并慢性肺部疾病的高海拔地区的普通型新型冠状病毒感染患者中,尝试一方面抢先给与高流量吸氧或无创,甚至有创正压通气氧疗,另一方面使用药物积极改善肺循环阻力,降低肺动脉压力治疗,改善患者心肺功能、缩短救治疗程、减少后遗症、改善预后。  

Objectives of Study:

COVID-19 is a new disease caused by severe acute respiratory syndrome. More and more studies have found that microthrombosis is a significant clinical feature of COVID-19. The coexistence of Pulmonary circulation microthrombosis and pulmonary exudation will lead to increased resistance of Pulmonary circulation and increased pulmonary artery pressure. There are about 80 million people living in high-altitude areas in China, and 5% -10% of the residents suffer from chronic high-altitude diseases due to their loss of high-altitude adaptability. High altitude pulmonary hypertension is a typical representative, which is characterized by moderate hypoxemia, presence or absence of right heart hypertrophy and right heart failure, and absence of Polycythemia. Therefore, after COVID-19 infection occurs in people living at high altitude, the cases with high altitude pulmonary hypertension, Polycythemia or COPD and other pulmonary diseases will further lead to increased Pulmonary circulation resistance, serious pulmonary hypertension and even pulmonary heart disease. We hope that among ordinary novel coronavirus infected patients with chronic lung diseases and severe, critical infective patients in high-altitude areas ,we can preemptively give high flow oxygen inhalation or non-invasive, even invasive positive pressure ventilation oxygen therapy, at the same time, we can use drugs to actively improve Pulmonary circulation resistance, reduce pulmonary artery pressure, improve patients' cardiopulmonary function, shorten the treatment course, reduce sequelae, and improve the prognosis.

纳入标准:

①年龄18岁-85岁;②入住医院重症医学科,诊断为新冠重型病例或合并心肺基础疾病的普通型病例(新冠肺炎诊断分型依据《新型冠状病毒肺炎诊疗方案(第九版)》);③患者签署知情同意书。

Inclusion criteria

① Age 18-85 years old; ② Check in the Intensive care medicine Department of the hospital, and diagnosed as COVID-19 severe, critical cases or ordinary cases with basic heart and lung diseases (COVID-19 pneumonia diagnosis and classification is based on the Diagnosis and Treatment Plan of novel coronavirus Pneumonia (Ninth Edition)); ③ The patient signs an informed consent form.

排除标准:

①患者存在正压氧疗相关禁忌症;②存在严重休克等使用改善肺循环药物的禁忌症;③孕妇或哺乳期妇女;④拒绝签署知情同意的患者。

Exclusion criteria:

① The patient has positive pressure oxygen therapy related Contraindication; ② serious shock and other Contraindication to use drugs to improve Pulmonary circulation; ③ Pregnant or lactating women; ④ Patients who refuse to sign informed consent.

本研究采取双向队列研究,均由参与研究经治医生收集前期新冠重型患者及合并心肺基础疾病的普通型新冠重症病例168例和后期应用积极正压氧疗联合药物改善肺循环治疗方案的相应类型新冠病例拟入组168例,但可能需要根据疫情发展情况调整.

This study adopts a two-way Cohort study, in which 168 cases of COVID-19 severe, critical patients and ordinary COVID-19 severe cases combined with basic heart and lung diseases in the early stage are collected by the doctors participating in the study, and 168 cases of COVID-19 disease of the corresponding type are proposed to be included in the treatment scheme of positive pressure oxygen therapy combined with drugs to improve Pulmonary circulation in the later stage, but may need to be adjusted according to the development of the epidemic situationcri
