研究目的：

1、探讨胸骨旁肋间肌增厚分数与膈肌增厚分数比值（TFic/TFdi）预测撤机结局的准确性，并与膈肌浅快呼吸指数比较明确其预测撤机结局的能力。 2、明确ICU机械通气患者基线呼吸肌肌肉厚度、肌肉厚度变化及呼吸肌功能与撤机结局的关系。  

Objectives of Study：

1. Our primary objective is to evaluate the performance of the ratio of parasternal intercostal muscle thickening fraction to diaphragmatic thickening fraction (TFic/TFdi) to predicti the weaning outcome ;and to compare the ability of TFic/TFdi and diaphragmatic rapid shallow breathing index to predict weaning outcome. 2. Our second objective is to determine whether baseline respiratory muscle thickness ,loss of muscle thickness and respiratory muscle function are associated with weaning outcome among mechanical ventilation patients in ICU

纳入标准：

①年龄≥18岁；②经口气管插管且预计ICU机械通气时间≥48h的患者；

Inclusion criteria

(a)Age≥18 years, (b)Patients intubated and mechanically ventilated for more than 48 h

排除标准：

①年龄＜18岁；②神经肌肉疾病、脊髓损伤；③妊娠；④首次SBT前48h内应用肌松剂；⑤气管切开、气胸、肋骨骨折、纵膈气肿、横膈解剖畸形或膈肌超声发现一侧膈肌功能异常者；⑥超声图像不清晰者

Exclusion criteria：

(a) age <18 years; (b) neuromuscular disease,Spinal cord injury;(c)pregnancy; (d) use of muscle-paralyzing agents within 48 h before first SBT; (e) presence of thoracostomy, pneumothorax, or flail chest or rib fractures ,pneumomediastinum and history or new detection of paralysis or paradoxical movement of a single hemidiaphragm on diaphragmatic ultrasonography; (f )unclear of the ultrasound image
