研究目的：

评价经皮等离子椎间盘减压术（PPDD）和脉冲射频术（PRF）通过下颈椎入路在治疗上肢根性疼痛、改善颈椎功能的同时，缓解上颈椎病变引起的颈源性头痛症状的有效性。  

Objectives of Study：

To evaluate the effectiveness of percutaneous plasma disc decompression (PPDD) and pulse radiofrequency(PRF) for relieving cervicogenic headache symptoms caused by upper cervical spine lesions through a lower cervical surgical approach ,while treating upper extremity radicular pain and improving cervical function.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 入院诊断为神经根型颈椎病且入院病史中颈椎症状及上肢症状明确。. 2. 主要体征和症状是单侧神经根性颈肩臂痛、上肢及手部的疼痛麻木等。 3. CT、MRI等检查显示，有颈椎间盘突出导致神经根受压的迹象。 4. 手术记录的治疗靶点为C5-C8颈神经（行PRF术）或C4-C7颈椎间盘（行PPDD术）。 5. 患者在社会心理学，精神、身体等层面上完全遵循本协议，包括遵循预定的访问，能完成理解量表内容。

Inclusion criteria

1. Admission diagnosis was cervical spondylotic radiculopathy with clear cervical and upper extremity symptoms in the admission history. 2. The main signs and symptoms were unilateral radicular pain, pain and numbness in the upper limbs and hands, etc. 3. CT and MRI of the cervical spine showed signs of nerve root compression due to cervical disc herniation. 4. The surgically documented therapeutic target is the C5-C8 nerve root (PRF) or C4-C7 disc (PPDD). 5. The patient is fully compliant with this protocol at the psychosocial, mental, and physical levels, including following scheduled visits and understanding the content of the scale.

排除标准：

1.合并游离椎间盘、严重椎管狭窄和纤维环钙化、后纵韧带骨化、腕管综合征、肩周炎。 2.合并截瘫或部分瘫痪、颈椎骨折或脱位、颈椎不稳。 3.在计划手术阶段有手术史，无法忍受治疗过程或有手术禁忌症。 4. 有明显社会心理疾病，沟通障碍的患者 5. 经研究者评估其他不适合参与研究的患者。

Exclusion criteria：

1. Combination of free disc, severe spinal stenosis and calcification of the fibrous annulus, ossification of the posterior longitudinal ligament, carpal tunnel syndrome, and frozen shoulder. 2. Combination of paraplegia or partial paralysis, cervical fracture or dislocation, and cervical instability. 3.History of surgery during the planned surgical phase, inability to tolerate the course of treatment or contraindications to surgery. 4. Patients with significant psychosocial disorders, communication disorders. 5.Other patients assessed by the investigator as unsuitable for participation.
