研究目的：

评估腺扁切除术治疗前青少年期儿童OSA的效果。  

Objectives of Study：

The purpose of this trial is to evaluate if preadolescent children with mild to moderate OSA benefit from AT compared to watchful waiting.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 入选标准 1) 前青少年期儿童（10≤年龄＜13）； 2) 诊断为非重度阻塞性睡眠呼吸暂停（轻度：1次/h10次/。诊断标准参照2020版中国儿童阻塞性睡眠呼吸暂停诊断与治疗指南[9]）； 3) 扁桃体肥大≥1度（Brodsky分度法[10]）。 0度：扁桃体位于扁桃体体窝内； 1度：扁桃体阻塞< 25%的气道； 2度：扁桃体阻塞25% - 50%的气道； 3度：扁桃体阻塞50% - 75%的气道； 4度：扁桃体阻塞≥75%的气道。 4) 患儿及患儿父母能充分理解试验过程，并完成问卷调查。

Inclusion criteria

? ≥10 to <13 years of age ? History or symptoms of OSA ? Mild to moderate OSA, defined as an obstructive apnea-hypopnea index (OAHI) of ≤10 ? Tonsil hypertrophy 1–4 according to Brodsky[10] ? Parents understand the written information and answer the questionnaires

排除标准：

2. 排除标准 1) 慢性扁桃体炎； 2) 极度肥胖（BMI z score ≥ 3）； 3) 重度OSA （OAHI>10 次/小时）； 4) 受试者有影响呼吸系统的其他疾病； 5) 多动症等其他可能影响多导睡眠检测结果的情况； 6) 受试者有染色体异常； 7) 受试者患有神经肌肉疾病 8) 心肺功能不全或其他手术禁忌； 9) 患有血友病或其他出血性疾病。 10) 既往已做过鼻、咽、喉等上呼吸道部位的手术，包括扁桃体切除术。

Exclusion criteria：

Recurrent tonsillitis ? Obesity (BMI z-score ≥ 3) ? Neuromuscular disease ? Chromosomal abnormality ? Previous adeno-tonsillar surgery ? Bleeding disorder ? Cardiopulmonary disease ? Conditions like hyperactivity disorder, etc., that may affect the results of polysomnography. ? Previous upper airway surgery on the nose, pharynx or larynx, including tonsillectomy.

区块随机法。随机准备60个密封的信封，腺扁切除术组和观察等待组的分配比例为1：1。每个区块有10个封信混合在一起（5个AT组、5个观察等待组），逐个区块使用。这些信封被放在包含研究信息和知情同意书的文件夹中，在接受研究参与后，儿童监护人和儿童一起打开。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The randomization is performed with 80 sealed envelopes and the allocation ratio is 1:1 between AT and watchful waiting. Five envelopes from each group are mixed together to create a block of 10. The envelopes are placed in folders containing study information and a consent form, and are opened together with the caregivers and children after accepting study participation.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Case Report Form (CRF): A standardized CRF will be used to record all relevant information for each participant. This will include demographic data, medical history, study-specific data, and any adverse events. The CRF will be completed by trained investigators or research coordinators. Electronic Data Capture (EDC): All data from the CRF will be entered into an electronic data capture system. This system will ensure accurate and efficient data entry and will allow for real-time data monitoring. The EDC system will be password protected, and access will only be given to authorized personnel. Data Validation: Data entered into the EDC system will be validated for accuracy and completeness. This will include range checks for data values and checks for missing data. Any discrepancies identified will be resolved before the data analysis. Data Security: All data will be stored securely to maintain participant confidentiality. This will include encryption of electronic data and secure storage of paper documents. All data will be anonymized or pseudonymized to ensure participant privacy. Data Analysis: The data will be analyzed as per the statistical analysis plan. Any changes to the plan will be documented and justified.
